摘要
目的探讨腹腔镜治疗胃溃疡穿孔的手术方式及临床优势。方法选择本院2013年5月至2015年6月接诊的胃溃疡穿孔患者90例为研究对象。随机分为两组,每组45例,对照组采取常规开腹手术,研究组则采取腹腔镜胃穿孔修补术治疗。观察记录两组患者手术时间、术中出血量、肛门排气时间、下床活动时间、胃管拔除时间、引流管拔除时间、住院时间,随访半年复发率。结果研究组患者手术时间长于对照组,但术中出血量、肛门排气时间、下床活动时间、胃管拔除时间、引流管拔除时间、住院时间方面要显著低于对照组(P<0.05);研究组随访半年无复发,对照组则有2例复发,差异无显著性(P>0.05)。结论胃溃疡穿孔采取腹腔镜穿孔修补术可明显减少术中出血量,缩短术后康复与住院时间,且安全性较高,值得推广。
Objective To investigate the surgical methods and clinical advantages of laparoscopic treatment of gastric ulcer perforation. Methods 90 cases of perforation of gastric ulcer patients in our hospital from May 2013 to June 2015 were selected as the research object and randomly divided into two groups, 45 cases in each group, the control group was treated with conventional laparotomy, the study group was treated with laparoscopic gastric perforation repair. Two groups of patients were observed and recorded operative time, blood loss, anal exhaust time, ambulation time, gastric tube removal time, the drainage tube removal time, hospital stay, and follow-up six months relapse rate. Results The results of the study groups in operative time was longer than that of the control group, but the amount of bleeding, anal exhaust time, ambulation time, gastric tube removal time, the drainage tube removal time, length of hospital stay was significantly lower than the control group; in the study group, there was no recurrence followed up six months, while there were two cases recurrence in the control group, the difference was not statistically significant （P 〉 0.05）. Conclusion The gastric ulcer perforation by laparoscopic repair surgery can significantly reduce the amount of bleeding, shorten the postoperative rehabilitation and hospitalization time and high safety, worthy of promotion.
