链霉素骨水泥椎体成形术治疗中心型单椎体结核疗效观察 被引量:4

Therapeutic effect of percutaneous vertebroplasty with streptomycin bone cement on central single vertebral tuberculosis

摘要 目的探讨经皮链霉素骨水泥注射椎体成形术治疗中心型单椎体结核的疗效。方法10例中心型单椎体结核患者,正规抗结核治疗≥2周,血沉≤40mm/h、C-反应蛋白≤30mg/L后行经皮链霉素骨水泥注射椎体成形术,术后继续正规抗结核治疗3周,随访观察治疗效果。结果手术过程顺利,术中2例发生骨水泥椎间盘渗漏,未出现骨水泥椎管内渗漏;术后第2天行X线片检查示骨水泥病椎内填充良好;术后5~7d病情稳定出院;出院后随访6个月,均未失访,术后i~3个月结核全身症状明显好转,术后6个月血沉及C-反应蛋白恢复至正常4例、接近正常6例,腰背部疼痛症状明显改善,X线片及CT、MRI检查均未见椎体塌陷及新的空洞形成、椎旁无脓肿。结论经皮链霉素骨水泥注射椎体成形术治疗中心型单椎体结核疗效满意、安全。 Objective To investigate the therapeutic effect of percutaneous vertebroplasty (PVP) by vertebral injection of streptomycin bone cement on central single vertebral tuberculosis. Methods Ten patients with central single vertebral tuberculosis received anti-tuberculosis treatment for ≥ 2 weeks. When erythrocyte sedimentation rate was ≤40 mm/h and C-reactive protein was ≤ 30 mg/L, the patients received pereutaneous vertebral injection of streptomycin bone cement The regular anti-tuberculosis treatment was continuous for 3 weeks after operation. Results The operations were carried on smoothly, with 2 cases of bone cement leakage into intervertebral disc and no leakage of bone cement into spinal canal. X-ray examination on the second day after operation showed bone cement filling well. The patients were discharged when the disease conditions were stable 5 to 7 days after operation. All patients were followed up for 6 months after being discharged. The general symptoms of tuberculosis were obviously relieved in 1 to 3 months after operation. The levels of erythrocyte sedimentation rate and C-reactive protein returned to normal in 4 patients and were close to normal in 6. Lumbar back pain symptoms were significantly alleviated. X-ray, CT and MRI examinations showed no vertebral collapse, new empty formation or vertebral abscess. Conclusion PVP by vertebral injection of streptomycin bone cement is effective and safe for central single vertebral tuberculosis.
