摘要
目的:研究牛磺熊去氧胆酸和熊去氧胆酸辅助多烯磷脂酰胆碱胶囊,对非酒精性脂肪性肝炎(NASH)患者血脂、肝功能指标及影像学指标的影响。方法:选取2012年1月至2015年6月在我院接受治疗的NASH患者110例,随机分为治疗组和对照组,每组各55例。两组患者均口服多烯磷脂酰胆碱胶囊2粒/次,3次/d,治疗组患者加用牛磺熊去氧胆酸250mg/次,3次/d;对照组患者加用熊去氧胆酸胶囊250mg/次,3次/d。观察疗程为48W。比较两组患者的疗效、肝功能指标、血脂水平和肝脏影像学检测情况。结果:对照组患者的总有效率为85.45%,治疗组患者为92.73%(且P<0.05)。治疗后两组患者的TBil、ALT、AST、GGT指标均明显下降,治疗组患者的TBil、ALT、AST、GGT均低于对照组(P<0.05);治疗后两组患者的TG、TC、LDL-C水平明显下降,HDL-C水平上升,治疗组患者的TG、TC、LDL-C水平均低于对照组,HDL-C水平高于对照组(P<0.05)。治疗后两组患者的脂肪肝影像学程度,治疗组患者改善优于对照组(P<0.05)。结论:牛磺熊去氧胆酸联合多烯磷脂酰胆碱胶囊可以明显改善NASH患者的肝功能和血脂水平,肝脏影像学检查结果也明显改善。疗效优于熊去氧胆酸胶囊与多烯磷脂酰胆碱联合组。
Objective：To study the effect of two kinds of ursodeoxycholic acid assisted polyene phosphatidylcholine on blood lipid,liver function and imaging improvement effect in NAFLD patients.Methods：110 cases of NAFLD patients who treated in our hospital were selected from January 2014 to June 2015,They were randomly assigned to treatment and control groups,each group had 55 cases,two groups of patients were given polyene phosphatidylcholine in the treatment,while the control group were treated with ursodeoxycholic acid,the treatment group were treated with tauroursodeoxycholic acid.Compared the two groups of curative effect,liver function index,blood lipid level and imaging situation.Results：The total effective rate of control group was 85.45%,while the treatment group was 92.73%（P〈0.05）.After treatment,TBil,ALT,AST,GGT liver function indexes of the two groups were significandy decreased,and the treatment group were lower than those of the control group（P〈0.05）;After treatment,the levels of TG,TC and LDL-C were significantly decreased in the two groups,while the levels of HDL- C increased;TC,TG and LDL- C in the treatment group were lower than those in the control group and HDL-C was higher than the control group（P〈0.05）.After treatment,the degree of fatty liver were significantly improved in the two groups,and the improvement of fatty liver in the treatment group was better than that in the control group（P〈0.05）.Conclusion：Tauroursodeoxycholic acid combined with polyene phosphatidylcholine can significantly improve the liver function,blood lipid levels and the imaging of NAFLD patients
