根除幽门螺杆菌对原发性肾小球肾炎患者蛋白尿的影响 被引量:3

摘要 目的研究原发性肾小球肾炎患者根除幽门螺杆菌(HP)后对患者蛋白尿的影响。方法选取2012年1月至2014年12月收治的86例经内镜活检诊断为胃黏膜HP感染的原发性肾小球肾炎患者为研究对象,随机将患者分为对照组(43例)和观察组(43例)。对照组患者采用原发性肾小球肾炎的常规治疗,观察组在对照组的基础上采用三联疗法对患者进行HP根除治疗。治疗前后检测患者血清中蛋白指标和尿蛋白指标,并分析治疗前后的差异。结果治疗后,两组患者总蛋白、白蛋白浓度均高于治疗前,肌酐、24 h尿蛋白量则显著低于治疗前,差异具有统计学意义(P<0.05);且观察组总蛋白、白蛋白、肌酐、24 h尿蛋白量变化更明显,差异具有统计学意义(P<0.05);两组患者治疗前后血清总胆固醇、甘油三酯、低密度脂蛋白、高密度脂蛋白比较无显著性差异(P>0.05)。结论幽门螺杆菌感染的原发性肾小球肾炎患者根除HP后,能有效改变患者血清肌酐、白蛋白和尿蛋白水平。 Objective To investigate the effect of Helicobacter pylori eradication on proteinuria in patients with primary glomerulonephritis. Methods The study was composed of 86 patients diagnosed with primary glomerulonephritis who had dyspeptic complaints throughout the diagnosis form January 2012 to December 2014. All these patients were randomly divided into control group (43 cases) and observation group (43 cases). Patients who received endoscopic biopsy upon the determination of pathologic findings in their upper gastrointestinal endoscopy were HP positive in their biopsy material. A triple eradication therapy was initiated for HP. The difference of serum protein and proteinuria was analyzed before and after treatment. Results After treatment, the total protein and albumin concentrations were higher than treatment before in both groups, while the creatinine and 24 h urinary protein excretion were significantly lower than treatment before in both groups, the statistically is significant difference ( P 〈0.05). The change of total protein, albumin, creatinine and 24 h urine protein in observation group is more obvious than control group, the statistically is significant difference ( P 〈 0 . 05). There were no significant difference in serum total cholesterol, triglyceride, low density lipoprotein and high density lipoprotein between the two groups before and after treatment ( P 〉 0 . 0 5 ) . Conclusion Patients with primary glomerulonephritis, HP eradication treatment has an effect on serum creatinine, serum albumin and proteinuria levels.
