下肢开放性骨折创口感染患者足甲襞微循环与血液黏度状态的变化观察

Changes of foot nailfold microcirculation and blood viscosity state in patients with open fracture of lower limb and wound infection

摘要 目的观察下肢开放性骨折创口感染患者足甲襞微循环及血液黏度状态的变化,为患者微循环的改善提供参考依据。方法选取2013年10月-2015年11月医院收治32例下肢开放性骨折创口感染患者为A组,同期选取32例下肢开放性骨折未发生创口感染患者为B组,32名体检健康者为C组,比较3组受试人员临床指标,数据采用SPSS 16.0软件进行统计分析。结果 A组的管襻数目、襻顶直径、输入枝长度及输出枝长度分别为(4.15±0.52)条/mm、(7.54±0.87)μm、(61.14±5.15)μm及(70.28±7.10)μm,均低于B组及C组,B组则低于C组,A组血液黏度指标均高于B组及C组,B组则高于C组,差异均有统计学意义(P<0.05)。结论下肢开放性骨折创口感染患者足甲襞微循环及血液黏度状态相对较差,感染的存在导致骨折患者的微循环状态进一步变差,因此应重视对感染的控制。 OBJECTIVE To observe the change situation of foot nailfold microcirculation and blood viscosity state in patients with open fracture of lower limb and wound infection,in order to provide a basis for the improvement of microcirculation state in patients.METHODS A total of 32 patients with open fracture of lower limb and wound infection in hospital from Oct.2013 to Nov.2015 were selected as group A.Another 32 patients with open fracture of lower limb but without wound infection at the same period were selected as group B,and 32 healthy personnel with the medical examination as group C.The clinical data of three groups were compared,and the results were analyzed by SPSS 16.0.RESULTS The capillary loop number,apical diameter,input branch length and output branch length of group A were respectively(4.15±0.52)branches/mm,(7.54±0.87)μm,(61.14±5.15)μm and(70.28±7.10)μm,and they were all lower than those in group B and C,group B even lower than group C.The blood viscosity indexes of group A were all higher than those of group B and group C,group B even higher than group C(P〈0.05).CONCLUSION The foot nailfold microcirculation and blood viscosity state of patients with open fracture of lower limb and wound infection are relatively worse,and the presence of infection causes worse microcirculation of patients with fracture,so the control of infection should be paid more attention.
