摘要
目的探讨腹腔镜根治手术治疗胃癌的临床疗效及对患者免疫功能的影响。方法将2007年1月~2014年12月来我院就诊的胃癌患者149例,随机分为研究组(n=88)和对照组(n=61)。研究组行腹腔镜根治手术,而对照组行传统开腹手术。观察两组患者的术中出血量,术后排气时间、住院时间及术后7d的免疫功能情况。结果研究组术中出血量明显少于对照组,术后排气时间明显早于对照组,住院时间明显短于对照组,差异比较均具有统计学意义(P<0.05)。此外,术后7d,研究组外周血CD3^+,CD4^+,CD8^+活性及IgG免疫球蛋白均高于对照组(P<0.05);术后1、3d对照组白细胞计数显著高于研究组(P<0.05)。结论腹腔镜根治手术能够促进胃癌患者术后恢复,同时保护患者免疫,使患者尽早出院。
Objective To investigate the clinical effect of laparoscopic radical operation in the treatment of gastric cancer and its influence on immune function of patients. Methods 149 cases of gastric cancer patients cured in our hospital from Jan 2007 to Dec 2014 were randomly divided into the research group（n=88） and the control group（n=61）. Patients in the research group were treated with laparoscopic radical operation, and patients in the control group were treated with conventional open operation. The amounts of bleeding during operation, postoperative exhaust time, hospitalization time, and the immune function after the operation for 7 days of the two groups were observed. ResultsThe amount of bleeding during operation of the research group was significantly less than that of the control group, and the postoperative exhaust time of the research group was earlier than that of the control group, and the hospitalization time of the research group was shorter than that of the control group, and the differences were all statistically significant（P0.05）. In addition, the CD3~＋, CD4~＋, CD8~＋ activity of the peripheral blood and the Ig G immunoglobulin level of the research group were all higher than those of the control group after the operation for 7 days（P 0.05）, and the white blood cell count of the control group were all significantly higher than those of the research group at the point of 1-day, 3-day（P 0.05）. Conclusion The therapy of laparoscopic radical operation is beneficial to the postoperative recovery of gastric cancer patients, and at the same time, it can help protect the immune function, thus shorten the discharge time.
