摘要
目的 探讨组织因子（TF）、组织因子途径抑制物（TFPI）在糖尿病并发脑梗死急性期的变化及其与短期预后的关系。方法 选择2012年8月~2014年1月浙江省4家医院682例糖尿病并发脑梗死患者为研究对象,研究终点事件定义为患者出院评分NIHSS≥5或住院期间死亡。采用非条件Logistic回归分析探讨TF、TFPI与糖尿病并发脑梗死患者短期预后的关系。结果 两组男性比例、年龄≥65岁比例、脑梗死分型、入院NIHSS评分和住院时间比较,差异均有统计学意义（P均〈0.05）;预后不良组TF、TFPI和TF/TFPI〉1比例均显著高于预后良好组,差异均有统计学意义（P均〈0.05）;调整性别、年龄、脑梗死分型、入院NIHSS评分和住院时间后,TF/TFPI〉1组发生终点事件的危险性是TF/TFPI≤1组的1.785倍（P〈0.05）。结论 糖尿病并发脑梗死患者急性期TF/TFPI是其预后不良的独立危险因素。
Objective To explore the relationship between tissue factor（TF）, tissue factor pathway inhibitor（TFPI） and the short term prognosis in patients with diabetic combined with cerebral infarction. Methods A total of 682 patients with diabetic combined with cerebral infarction from four hospitals of Zhejiang province were included into the study from August 2012 to January 2014. Endpoint of the study was defined as poor short term prognosis（NIHSS≥5） at discharge or death during hospitalization. Non-conditional logistic regression analysis was used to evaluate the association between TF, TFPI and short term prognosis in patients with diabetic cerebral infarction. Results There were significant differences of the sex, age≥65 years, cerebral infarction type, NIHSS score on admission and hospitalization time between the two groups（P0.05）. The TF, TFPI, TF/TFPI1 in the poor prognosis group were significantly higher than that in the good prognosis group（P0.05）; After adjusted sex, age, cerebral infarction type, NIHSS score on admission and hospitalization time, the incidence rate of endpoint in the TF/TFPI1 group was 1.785 times than that in the TF/TFPI≤1 group（P0.05）. Conclusion The level of TF/TFPI at admission is independent risk factor of poor short term prognosis in patients with diabetic combined with cerebral infarction.
