摘要
目的探讨maxMorespine椎间孔镜治疗老年腰椎间盘突出症的临床效果。方法将100例老年腰椎间盘突出症患者按治疗方法不同分为2组:对照组50例给予椎板间开窗髓核摘除术治疗,观察组50例给予经皮maxMorespine椎间孔镜椎间盘切除术治疗。比较2组手术临床指标(包括手术时间、术中出血量、卧床时间、住院时间、间盘切除量及咬骨体积等)及术前术后VAS疼痛评分与ODI功能障碍指数改善情况、术后并发症发生情况与临床疗效等。结果观察组术中出血量、卧床时间、住院时间、间盘切除量及咬骨体积均明显低于对照组(P<0.05);2组术后VAS疼痛评分及ODI功能障碍指数均较术前明显降低(P<0.05),2组术后VAS疼痛评分及ODI功能障碍指数比较差异无统计学意义(P>0.05);观察组术后并发症发生率明显低于对照组患者(P<0.05),临床疗效优良率明显高于对照组(88.0%比76.0%,P<0.05)。结论经皮maxMorespine椎间孔镜治疗老年腰椎间盘突出症临床疗效显著,创伤小,并发症发生率低。
Objective To evaluate the clinical efficacy of maxMorespine system in the treatment of lumbar disc herniation in elderly patients.Methods One hundred elderly patients with lumbar disc herniation were treated with either fenestration discectomy（control group,n=50）or percutaneous maxMorespine discectomy（observation group,n=50）.Operation time,intraoperatve blood loss,bed rest time,hospital stay,amount of disc removed,volume of bone bitten,and other clinical parameters were compared between the two groups.Furthermore,VAS pain score and ODI dysfunction index were measured before and after operation.Moreover,postoperative complications and clinical efficacy were observed in both groups.Results Compared with control group,intraoperatve blood loss,bed rest time,hospital stay,amount of disc removed,volume of bone bitten and incidence of postoperative complications decreased in observation group（P0.05）.VAS pain score and ODI dysfunction index decreased after operation in both groups（P0.05）,but the differences were not significant between the two groups（P0.05）.The rate of good clinical outcome in observation group was higher than that in control group（88.0% vs 76.0%,P0.05）.Conclusion Percutaneous maxMorespine discectomy is effective for lumbar disc herniation and causes less trauma and fewer complications in elderly patients.
