摘要
目的探讨超声引导下颈内静脉穿刺置管术在基层医院重症医学科(ICU)临床应用价值。方法选取2015年4月至2016年2月需行颈内静脉穿刺置管的100例重症患者为观察对象,通过随机方法分为超声引导组与传统方法组,每组各50例。观察两组患者在穿刺置管成功率、置管时间和并发症发生率等情况。结果两组患者穿刺成功率和穿刺次数比较超声方法组明显优于传统方法组(P〈0.05),一次成功率更明显(P〈0.01),置管时间超声方法组也较传统方法组少(P〈0.01);两组并发症发生情况比较总发生率超声方法组较传统方法组明显减少,差异有统计学意义(P〈0.05)。穿刺损伤动脉、血肿、气胸、血胸、导管异位、导管相关性血行感染两组间比较差异无统计学意义(P〉0.05)。结论超声引导下颈内静脉穿刺置管术在基层医院ICU使用是安全、可行的,超声引导可以明显提高穿刺成功率,尤其是一次穿刺成功率,减少穿刺使用时间及总体并发症的发生率。
Objective To investigate the clinical effect of ultrasound guided internal jugular vein catheterization in the ICU application in basic hospital. Methods Cases were selected from those had critical care medication in our hospital from April 2015 to February 2016 who needed to line the internal jugular vein catheterization 100 cases of critically ill patients as research subjects, by the method of randomly dividing into guided by ultrasound group and the traditional method, observe two groups of patients in the success rate of tube, catheter time and complication rates. Results The puncture success rate and puncture times of the ultrasound group were significantly better than that of the traditional method group ( P〈0.05 ) .One time puncture rate was of more significant difference ( P〈0.001 ) , the time of insertion ultrasonic method group also relatively less traditional Methods group ( P〈0.001 ) ; for the complications incidence, the ultrasonic method decreased significantly ( P〈0.05 ) , the difference was statistically significant.Puncture artery injury, hematoma, pneumothorax, hemothorax, catheter ectopia, catheter-related blood line infection between the two groups is not statistically significant difference ( P〉0.05 ) . Conclusion Ultrasound guided internal jugular vein puncture catheter used in the primary hospital ICU is safe and feasible, ultrasound guidance can significantly improve the success rate of ptmcture, especially for one time puncture success rate, reduce the puncture using time and the total complication incidence.
