摘要
目的探讨以患者和其照顾者的需求为导向的护理措施对脑卒中患者及其照顾者的应用效果。方法 选取2013年4月-2015年4月本院收治的132例脑卒中患者,按照随机原则分为对照组和观察组,各66例。两组患者出院前均行神经内科常规护理指导。观察组患者和其照顾者行需求评估,根据其需求在常规护理的基础上,采用以需求为导向的护理措施进行干预2个月,对照组的照顾者则仅采用常规护理方法,于患者出院后的1周、2周、1个月以及2个月时,比较分析两组患者日常生活能力、生活质量和对护理的总满意度,并评价两组照顾者的自我效能和生活质量。结果 出院后,观察组患者日常生活能力、生活质量和护理满意度均明显高于对照组（P〈0.05）;且随时间的增加,其日常生活能力、生活质量和护理满意度评分均增高;观察组患者的照顾者在出院后的生活质量和自我效能均明显高于对照组（P〈0.05）。结论 以患者及其照顾者的需求为导向的护理干预措施有利于提高脑卒中患者康复中的生活质量,值得临床推广应用。
Objective To explore the intervention effects of nursing measures that with the needs of patients and their caregivers as a guide in stroke patients and caregivers.Methods 132 stroke patients from April 2013 to April 2015 of our hospital were selected and were divided into control group and observation group according to random principle.Each group was 66 cases.Routine nursing instruction in nerve department of internal medicine was conducted in patients in two groups before discharge.Needs assessment were conducted in observation group patients and caregivers,and then on the basis of routine nursing care according to their need.At the same time,demand oriented nursing measures were used in observation group patients and caregivers for intervention 2 months.The caregivers in control group were only used routine nursing method.1 week,2 weeks,1 month and 2 months after patients discharged,the daily life ability,life quality and overall satisfaction with care in two groups were compared.Self efficacy and life quality of caregivers in two groups were evaluated.Results After patients discharged,the daily life ability,life quality and nursing satisfaction in observation group were obvious higher than that of control group（P〈0.05）;And with the increase of time,the score of daily life ability,life quality and nursing satisfaction of patients increased;Self efficacy and life quality of caregivers in observation group were obvious higher than that of control group after patients discharged（P〈0.05）.Conclusion Nursing interventions on the needs of patients and their caregivers as a guide are helpful in enhancing the life quality of stroke patients and is worth to popularize in clinical.
