摘要
目的探讨复杂型距舟关节骨折脱位的临床治疗效果。方法采用手术治疗53例复杂型距舟关节骨折脱位患者,在连续硬膜外麻醉下,根据骨折脱位分型在C臂X线机引导下实施相应的内固定手术,在复位满意后,对合并软组织伤进行有效处理。做好术后抗感染、功能恢复训练等工作。术后随访3~6个月。结果患者均手术成功,平均随访（4.1±0.5）个月,优19例（35.8%）,良28例（52.8%）,可4例（7.5%）,差2例（3.8%）,优良率为88.7%（47/53）。术后手术切口感染1例,治疗后治愈。结论复杂型距舟关节骨折脱位通过积极有效的手术,进行解剖复位,可最大限度保证血运,促进关节功能恢复,临床效果显著。
Objective To explore the clinical effect of surgical treatment in complex fracture and dislocation of articulatio talonavicularis. Methods A total of 53 cases with fracture and dislocation of articulatio talonavicularis were treated with internal fixation surgery in the condition of continuous epidural anesthesia,under the guide of C-arm X-ray machine. After reduction,the damage of the soft tissue was treated accordingly,than after the operation,the intervention of anti infection and functional recovery training were given. The patients were followed up for 3 to 6 months. Results Operations were successfully performed in all 53 cases. The average follow-up time were（ 4. 1 ± 0. 5） months,with excellent in 19 cases（ 35. 8%）,good in 28 cases（ 52. 8%）,comnon in 4 cases（ 7. 5%）,poor in 2 cases（ 3. 9%）,the rate of excellent and good was 88. 6%（ 47 /53）. Postoperative incision infection occurred in 1 case,and healed after treatment.Conclusion Through the anatomical reduction of complex fracture and dislocation of articulatio talonavicularis,the joint can get good blood circulation,and promote the recovery of joint function. Its clinical effect is remarkable.
