能谱CT碘基物质分析对鼻咽癌临床诊断的价值 被引量:5

摘要 目的 CT能谱成像(GSI)技术对鼻咽癌诊断的临床价值。资料与方法选取经鼻咽活检病理证实的92例鼻咽部病变患者(其中35例鼻咽癌为实验组,57例鼻咽炎症为对照组)。采用能谱CT对所有患者进行GSI单能模式增强扫描,测量并计算鼻咽部病变组织的碘浓度和碘浓度比,并与病理结果对照,计算能谱CT对鼻咽癌诊断的准确率、敏感性及特异性。结果实验组与对照组的碘浓度(100μg/m1)分别为,动脉期:13.16±3.23、9.69±2.86和静脉期:20.53±4.14、13.49±3.67,碘浓度比分别为动脉期:0.145±0.04、0.104±0.03和静脉期0.329±0.12、0.232±0.08。在动脉期,实验组与对照组的碘浓度和碘浓度比两两比较差异有统计学意义(P均<0.001);静脉期时,实验组与对照组的碘浓度和碘浓度比两两比较差异有统计学意义(P均<0.001)。能谱CT对鼻咽癌诊断的准确率、敏感性及特异性分别为95.65%、94.29%及96.49%。结论能谱CT对鼻咽癌的诊断具有重要价值。 Objective To discuss the clinical value of spectral CT imaging(GSI) technology on diagnosis of nasopharyngeal carcinoma. Materials and Methods Totally 92 patients with nasopharyngeal lesions confirmed by nasopharyngeal biopsy pathology(among them, 35 cases with nasopharyngeal carcinoma as experimental group, 57 cases with nasopharyngitis as control group)underwent CT imaging with GSI mode and the data were retrospectively analyzed. The iodine concentration,normalized iodine concentration of lesions were measured and calcultated,and the accuracy, sensitivity and specificity of this scanning technology were calcultated.AII results were compared with pathology. Results The iodine concentration of experimental group and control group was13.16±3.23、9.69±2.86(100μg/m1) in arterial phase,and 20.53±4.14、13.49±3.67(100 μg/m1) in venous phase,respectively. NIC was0.145±0.04、0.104±0.03 in arterial phase and 0.329±0.12、0.232±0.08 in venous phase,respectively.There were significant differences between experimental group and control group on iodine concentration and NIC in arterial phase(all P〈0.001), there were significant differences between experimental group and control group on iodine concentration and NIC in venous phase(all P 0.001). The accuracy, sensitivity and specificity of this scanning technology were 95.65%, 94.29% and 96.49%, respectively. Conclusion CT gemstone spectral imaging has a important value to making a diagnosis of nasopharyngeal carcinoma
