钢板加异体骨板治疗股骨干钢板内固定术后骨不连 被引量:4

Treatment of nonunion of femoral shaft fracture after initial plate fixation with plate and cortical allograft struts

摘要 目的:探讨钢板加异体骨板治疗股骨干钢板内固定术后骨不连的临床疗效。方法对19例股骨干骨折钢板内固定术后骨不连患者采用钢板加对侧异体骨板夹心法手术治疗,术后1年采用Merchan标准评定膝关节功能。结果患者切口均一期愈合,无脂肪栓塞、感染、排异等反应。19例均获得随访,时间14~44个月。患者术后均达到骨性愈合,时间为4~9个月。无内固定失败、畸形愈合和不愈合等发生。术后1年膝关节功能按Merchan 标准评定:优11例,良6例,可2例。结论钢板加对侧异体骨板是治疗股骨干骨折钢板内固定术后骨不连的有效方法。 Objective To investigate the clinical effects of plate and cortical allograft struts for the treatment of non -union of femoral shaft fracture after initial fixation with plate .Methods Nineteen cases with nonunion of femoral shaft fracture after initial fixation with plate were treated with femoral plate and cortical allograft struts .The clinical outcomes of the treatment were evaluated by Merchan assessment in one year postoperation .Results All of the pa-tients were primary healing and no complications such as fat embolism ,infection ,rejection were occurred .All the cases were completed review with 14-44 months.All of the patients had achieved bone healing in 4-9 months postopera-tively.No complications such as internal fixation breaking ,malunion and nonunion were occurred .According to the criterion of Merchan ,the results were excellent in 11 cases,good in 6,fair in 2.Conclusions It is an effective meth-od that using plate and cortical allograft struts to treat nonunion of femoral shaft fracture after initial plate fixation .
