缬沙坦+饮食控制对高血压合并糖耐量降低患者胰岛素抵抗的影响 被引量:6

摘要 目的 探讨缬沙坦+饮食控制对高血压合并糖耐量降低患者胰岛素抵抗情况的影响。方法 收集我院2014年7-12月门诊诊断为高血压合并糖耐量降低的患者100例,随机将其分为两组,每组各50例,其中缬沙坦组给予缬沙坦80 mg,1次/d,缬沙坦+饮食控制组在给予缬沙坦80 mg,1次/d的同时,严格饮食控制,共治疗6个月。观察两组患者治疗前后血压、空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR)的变化。结果 两组治疗后血压较治疗前相比均得到有效控制(P〈0.01),缬沙坦组FBG治疗前:(6.14±0.50)mmol/L vs治疗后:(6.10±0.44)mmol/L,2 h PBG治疗前:(9.27±0.75)mmol/L vs治疗后:(9.19±0.56)mmol/L,FINS治疗前:(14.93±1.07)m U/L vs治疗后:(14.89±0.98)m U/L,HOMA-IR治疗前:(4.09±0.54)vs治疗后:(4.05±0.47),差异均无统计学意义(P=0.196,P=0.128,P=0.124,P=0.085)。缬沙坦+饮食控制组FBG治疗前:(6.10±0.52)mmol/L vs治疗后:(5.45±0.43)mmol/L,2 h PBG治疗前:(9.38±0.84)mmol/L vs治疗后:(8.03±0.59)mmol/L,FINS治疗前:(14.77±1.13)m U/L vs治疗后:(13.14±1.02)m U/L,HOMA-IR治疗前:(4.02±0.55)vs治疗后:(3.20±0.44),差异均有统计学意义(均P〈0.01)。结论 缬沙坦对高血压合并糖耐量降低患者的胰岛素抵抗无明显影响。 Objective To explore the influence of Valsartan±diet control on insulin resistance in patients with hyper- tension combining with impaired glucose tolerance. Methods From July to December in 2014,100 patients with hyper- tension and impaired glucose tolerance were evenly divided into two groups in random.In Valsartan group,80 mg Val- sartan once per day was provided,while in Valsartan±diet control group,besides 80 mg Valsartan once per day,strict control on diet was needed.The course of treatment was lasted for 6 months.The blood pressure,fasting blood glucose (FBG),two-hour postprandial blood glucose (2 h PBG),fasting insulin (FINS) and homeostasis model assessment of in- sulin resistance index (HOMA-IR) changes before and after treatment in the two groups were observed. Results The blood pressure was effectively controlled after treatment in comparison with those before treatment in both groups (P〈0.01). The FBG,2 h PBG,FINS,and HOMA-IR in the Valsartan group were not displayed statistical differences before and af- ter treatment.To be specific,the values of FBG,2 h PBG,FINS,and HOMA-IR before and after therapy in the Valsartan group were (6.14±0.50) mmol/L and (6.10±0.44) mmol/L (P=0.196),(9.27±0.75) mmol/L and (9.19±0.56) mmol/L (P= 0.128),(14.93±1.07) mU/L and (14.89±0.98) mU/L (P=0.124),and (4.09±0.54) and (4.05±0.47) (P=0.085).In the Valsar- tan±diet control group,these indexes were greatly improved after therapy compared with those before,which were dis- played as follows: (6.10±0.52) mmol/L and (5.45±0.43) mmol/L (P〈0.01),(9.38±0.84) mmol/L and (8.03±0.59) mmol/L (P〈0.01),(14.77±1.13) mU/L and (13.14±1.02) mU/L (P〈0.01),and (4.02±0.55) and (3.20±0.44) (P〈0.01) in turn. Con- clusion Valsartan has no significant influence on insulin resistance in hypertensive patients with impaired glucose tolerance.
