摘要
目的：分析和探讨中鼻道双侧鼻窦对称性开放技术对鼻通气功能的影响。方法：选取我院2012年12月~2013年12月进行鼻内镜手术治疗阻塞性睡眠呼吸暂停低通气综合征（OSAHS）的患者60例作为研究对象,按照病情程度分为中度患者组（n=39）,重度患者组（n=21）。中度患者接受鼻中隔偏曲矫正＋鼻甲射频消融术治疗,重度患者采用鼻中隔偏曲矫正＋鼻甲射频消融＋中鼻道双侧鼻窦对称性开放术治疗,对两组患者手术前后主观症状进行观察和分析,比较鼻腔阻力及鼻容积变化。结果：（1）鼻阻力检查可为OSAHS患者主观症状的改变提供客观依据;（2）两种不同术式下患者术前、术后鼻腔阻力、鼻容积、视觉模拟评分法（VAS评分）差异显著;（3）两种术式之间鼻阻力差异显著（〈0.05）。结论：（1）鼻阻力测定可作为鼻通气术前后鼻通气功能的检测手段,适用于OSAHS患者,有利于客观评估患者手术前后鼻阻症状改善程度;（2）对于OSAHS患者,鼻中隔偏曲矫正＋鼻甲射频消融＋中鼻道双侧鼻窦对称性开放术治疗效果比鼻中隔偏曲矫正＋鼻甲射频消融术疗效更胜一筹,能明显改善鼻腔通气功能,加快患者康复速度,具有重要的临床价值。
Objective： to analyze and study the effect of nasal sinuses symmetry of bilateral open nasal ventilation function.Methods： in our hospital from 2012 December ~2013 year December the nasal endoscopic operation for treatment of obstructive sleep apnea hypopnea syndrome in 60 patients as the research object,according to the severity of disease was classified as moderate group（n=39）,severe group（n=21）, severe patients received deviation of nasal septum and inferior turbinate radiofrequency ablation treatment of patients with moderate,with deviation of nasal septum and inferior turbinate radiofrequency ablation and middle meatus of nose paranasal sinuses symmetry operation to opentreatment, observation and analysis of subjective symptoms in two groups of patients before and after operation,the volume of nasal cavity and nasal resistance change.Results：（1） to provide an objective basis of nasal resistance check for OSAHS patients subjective symptoms change;（2） of two different surgical patients after operation and postoperative nasal cavity nasal resistance,volume,VAS score was significantly different（3）;two were significant difference between the nasal airway resistance（ 0.05）.Conclusion：（1） the determination of detection means can be used as a nasal ventilation function before and after nasal surgery of nasal resistance,suitable for OSAHS patients,is conducive to the objective evaluation of patients before and after operation of nasal obstruction symptoms;（2）for patients with OSAHS,the deviation of nasal septum and inferior turbinate radiofrequency ablation and middle meatus of nose paranasal sinuses symmetry opening treatment effect of deviation of nasal septum and inferior turbinate radiofrequency ablation therapeutic effect is better, can significantly improve the nasal ventilation function,accelerate rehabilitation speed,it has important clinical value.
