摘要
目的评价超声引导下针刀松解椎间孔外口治疗神经根型颈椎病的临床疗效。方法选取60例神经根型颈椎病患者随机数字表法分为超声组和非超声组(n=30),超声组在超声引导下行椎间孔外口针刀松解+神经阻滞(U,n=30);非超声组在传统方法下行椎间孔外口针刀松解+神经阻滞((N,n=30)。观察记录治疗前、治疗后1、2、4和8周疼痛视觉模拟评分(VAS);记录治疗次数≥3的例数;记录治疗后8周疗效评定及不良反应发生情况。结果两组患者治疗前与治疗后相比,疼痛均有明显改善(P<0.05);治疗后1周、2周、4周和8周VAS评分U组均低于N组(P<0.05);U组治疗次数≥3的例数明显少于N组,8周疗效评定U组均优于N组(P<0.05);两组患者治疗中及治疗后不良反应发生率,差异无统计学意义。结论超声引导下椎间孔外口针刀松解治疗神经根型颈椎病疗效显著,是一种安全、可靠的可视化治疗技术。
Objective To evaluate the clinical effect of acupotomy lysis inexternal opening the intervertebral foramen for cervical radiculopathy by ultrasound-guidance. Methods Sixty patients with cervical radiculopathy were randomly divided into the ultrasound group（group U） and the non-ultrasound group（group N）,（n=30 in each group）：patients in group U were treated by ultrasound-guided acupotomy lysis inexternal opening the intervertebral foramen combine with nerve block, patients in group N were treated by acupotomy lysis combine with nerve block in the traditional way.Visual analogue scale（VAS） was recorded before treatment and at 1,2,4,8 weeks after treatment. Efficacy evaluation were recorded at8 weeks after treatment.The number of cases with treatment times≥3 were recorded.Incidence of adverse reactions were also recorded after treatment.Results Pain was obviously relieved after treatment in both groups（P 〈0.05）. VAS score at 1,2,4,8 weeks after treatment were decreased in two groups（P〈0.05）. Effective evaluation in group U at 8 weeks after treatment was better than that in the group N（ P〈0.05）. The number of cases with treatment times≥3 in group U were significantly less than that in group N. There was no significant difference in incidence of adverse reactions after treatment between two groups. Conclusion Compared with the traditional technique of needle knife therapy,acupotomy lysis inexternal opening the intervertebral foramen for Cervical radiculopathy under ultrasound-guidance has higher accuracy and better curative effect, which appear to be a safe and effective visualized therapeutic method.
