胸水腺苷脱氨酶检测在结核性胸腔积液诊断中的价值

摘要 目的:探讨血清和胸水腺苷脱氨酶(ADA)检测在胸腔积液鉴别诊断中的价值。方法:选取笔者所在医院2011年1月-2014年6月收治的73例胸腔积液患者,其中52例结核性胸腔积液患者作为结核组,21例癌性胸腔积液患者作为癌性组,所有患者均进行胸水ADA的定量分析,并进行统计学分析。结果:结核组的胸水ADA值为(48.6±15.8)U/L,明显高于癌性组的(11.5±6.2)U/L,差异有统计学意义(t=10.408,P=0.000)。当ADA≥20 U/L时,结核组的阳性率为94.2%;癌性组阳性率为14.3%,最高值为24.5 U/L。结论:胸水ADA是鉴别结核性胸腔积液与癌性胸腔积液的良好指标。 Objective:To investigate the value of detection of adenosine deaminase(ADA) in serum and hydrothorax for the identification and diagnosis of pleural effusion.Method:From January 2011 to June 2014,73 patients with pleural effusion admitted to our hospital were selected,52 patients with tuberculous pleural effusion were regarded as the tuberculosis group,and 21 patients with malignant pleural effusion were regarded as the carcinomatous group.All patients were given quantitative analysis of ADA in pleural effusion,and statistical analysis was conducted.Result:The ADA of hydrothorax in the tuberculosis group was(48.6±15.8)U/L,which was significantly higher than(11.5±6.2)U/L in the carcinomatous group,the difference was statistically significant(t=10.408,P=0.000).When ADA was equal to or larger than 20 U/L,the positive rate of tuberculosis group was 94.2%,that of carcinomatous group was 14.3%,the highest value was 24.5 U/L.Conclusion:Pleural effusion ADA is a good indicator for the differential diagnosis of tuberculous pleural effusion and malignant pleural effusion.
