摘要
目的观察自拟解毒抗癌经验汤剂、西妥昔单抗联合化疗对晚期结肠癌患者免疫功能及生活质量的影响。方法将94例晚期结肠癌患者随机均分为两组。对照组给予FOLFIRI方案化疗,观察组则加用自拟解毒抗癌经验汤剂、西妥昔单抗治疗。观察近期临床疗效及用药安全性,对比两组治疗前后的疼痛NRS评分及生活质量KPS评分,检测患者血清癌胚抗原(CEA)、炎性因子水平变化,检测免疫功能相关指标T淋巴细胞亚群的水平变化。结果观察组的近期疗效显著优于对照组,其疼痛NRS评分显著低于对照组,而生活质量KPS评分则显著高于对照组,差异具有统计学意义(P<0.05);观察组CEA、IL-1β、IL-6水平经治疗后均显著降低,明显低于对照组,差异具有统计学意义(P<0.05);观察组与机体免疫相关的CD8^+水平显著低于对照组,CD4^+、CD4^+/CD8^+水平显著高于对照组,差异均具有统计学意义(P<0.05);观察组胃肠不适、神经毒性、白细胞下降3方面的不良反应显著低于对照组,差异显著(均P<0.05),其余不良反应无显著差异(P>0.05)。结论采用中西医结合联合化疗法有利于减轻晚期结肠癌患者的疼痛,提高生活质量,减毒增效作用突出,其机制与提高机体免疫功能、调控肿瘤因子CEA水平有一定相关性。
Objective To research on the influence of Jiedu Kang＇ai decoction combined with Cetuximab on immune function and life quality of patients with advanced colorectal cancer. Methods 94 patients with advanced colorectal cancer were randomly divided into control group and observation group,47 cases in each group. Control group was given FOLFIRI scheme of chemotherapy; observation group was given self- made Jiedu Kang＇ai decoction combined with Cetuximab. Observe the recent curative efficacy and safety,compare the NRS score and KPS score,test the levels of CEA,IL- 1βand IL- 6,observe the levels of T lymphocyte subsets changes associated with the patients＇ immune function before and after treatment. Results The recent curative efficacy of observation group was 53. 2%,higher than 23. 4% of the control group,the pain NRS score was lower than that of the control group,the KPS score was higher than that of the control group,the differences were statistically significant（ P〈0. 05）; the levels of CEA,IL- 1β and IL- 6 in observation group were lower than those of the control group,the differences were statistically significant（ P〈0. 05）; the level of CD8~＋in observation group was lower than that of the control group,while the levels of CD4~＋,CD4~＋/ CD8~＋were higher than those of the control group,the differences were statistically significant（ P〈0. 05）. The adverse reactions of gastrointestinal discomfort,neurotoxicity and white blood cells decline in observation group were lower than those of the control group,the differences were statistically significant（ P〈0. 05）,but the difference of other adverse reactions was without significance（ P〉0. 05）. Conclusion Jiedu Kang＇ai decoction combined with Cetuxionab can reduce toxicity and enhance efficacy,is effective to ease the pain of patients with advanced colon cancer,thus to improve the quality of life. The mechanism may be associated with improving the patients＇ immunity and regulating the tumor factors CEA levels.
