冲击波联合综合康复治疗对下肢烧伤后增生性瘢痕的疗效观察 被引量:22

摘要 目的:观察冲击波疗法(SWT)联合综合康复治疗对下肢烧伤后增生性瘢痕的疗效。方法:32例下肢烧伤后增生性瘢痕患者随机分为观察组和对照组各16例。对照组进行常规康复治疗,观察组同时还进行SWT。治疗前后,采用温哥华瘢痕量表(VSS)评估瘢痕,视觉模拟量表(VAS)评估疼痛、下肢关节活动度、膝关节伸展与屈曲肌群等速肌力和6min步行能力测试(6MWT)对患者进行评估。结果:治疗12周后,2组患者的VSS评分、VAS评分、ROM、膝关节伸展、屈曲肌群等速肌力和6MWT步行距离均较治疗前明显进步(P<0.05),观察组VSS评分更优于对照组(P<0.05),而VAS评分、ROM、等速肌力和6MWT距离则均无统计学差异。结论:本研究初步证明冲击波联合综合康复治疗对烧伤后增生性瘢痕的患者有一定临床疗效,值得在临床上推广应用,但其机制仍需进一步研究。 Objective: To investigate the effectiveness of shockwave therapy (SWT) combined with comprehensive rehabilitation therapy to treat the post-burn hypertrophic scar. Methods: Thirty-two cases of hypertrophic scar after burn were randomly divided into the treatment group and the control group with 16 cases in each group. The partici pants in the control group only received the traditional rehabilitation therapy, and the subjects in the treatment group were given SWT combined with the same treatments as control group. The outcome measurements included the Vancouver Scar Scale (VSS) to assess the hypertrophic scar, pain (VAS), range of motion (ROM) of lower limbs, isokinetic muscle strength of knee extension and flexion muscles, and 6 rain walk test (6MWT). Results: After 12 weeks of treatment, the VSS scores in the treatment group were lower than those in control group (P〈0.05), but the VAS, ROM, muscle strength, and 6MWT showed no significant difference between two groups. Conclusion. SWT combined with comprehensive rehabilitation therapy can effectively inhibit the growth Of post-burn hypertroph ic scar in lower extremity. It can be applied to clinical practice.
