甲状腺结节超声应变弹性成像定量分析对结节良恶性的判断价值 被引量:17

摘要 目的:研究甲状腺结节超声应变弹性成像定量分析对结节良恶性的判断价值。方法:选择在我院诊断为甲状腺结节且接受超声应变弹性成像定量分析的患者进行研究,根据细针穿刺活检或手术切除后的病理结果分为良性组和恶性组;测定超声应变弹性成像的量化指标应变均值、蓝色区域面积、杂乱度,结节组织中恶性生物学分子的表达量以及血清中肿瘤标志物的含量。结果:恶性组的应变均值低于良性组,蓝色区域面积、杂乱度高于良性组,标准偏差、复杂度、峰度、偏度、对比度、均等性、一致性、相关性无显著差异;恶性组甲状腺结节中Fascin-1、S100A4、STAT3、TC-1、MUC1、MUC15的mRNA含量以及血清中Midkine、Galectin-3、CEACAM1、TFF3的含量显著高于良性组且与应变均值呈负相关,与蓝色区域面积、杂乱度呈正相关。结论:甲状腺结节超声应变弹性成像的量化指标应变均值、蓝色区域面积、杂乱度能够判断结节良恶性、评估恶性程度。 Objective:To analyze the value of thyroid nodule ultrasonic strain elastography quantitative analysis in judging benignancy or malignancy of nodules.Methods:Patients diagnosed as thyroid nodule who received ultrasonic strain elastography quantitative analysis in our hospital were selected for study and divided into benign group and malignant group according to fine needle biopsy or the pathological results after surgical resection;mean strain values of quantitative indicators,blue region area and disorder of ultrasonic strain elastography,expression levels of malignant biological molecules in nodule tissue as well as the contents of serum tumor markers were detected.Results:Mean strain value of malignant group was lower than that of benign group,blue region area and disorder were higher than those of benign group.But no significant difference in standard deviation,complexity,kurtosis,skewness,contrast,equality,consistency was found and no correlations were found.mRNA contents of Fascin-1,S100A4,STAT3,TC-1,MUC1 and MUC15in thyroid nodules as well as serum Midkine,Galectin-3,CEACAM1 and TFF3contents of malignant group were significantly higher than those of benign group,negatively correlated with mean strain value and positively correlated with blue region area and disorder.Conclusion:Mean strain values of quantitative indicators,blue region area and disorder of thyroid nodule ultrasonic strain elastography can judge benign or malignant nodules and assess the malignant degree.
