犀角地黄汤对脓毒症大鼠肝功能及肝组织病理的影响 被引量:20

Effect of Xijiao Dihuang Tang(犀角地黄汤) on Liver Function and Liver Tissue Pathology of Rats with Sepsis

摘要 目的观察犀角地黄汤对脓毒症大鼠肝脏的保护作用,并探讨其可能作用机制。方法 48只SD大鼠随机分为假手术组、模型组和犀角地黄汤组各16只。犀角地黄汤组和模型组大鼠采用盲肠结扎穿孔术制作脓毒症模型。犀角地黄汤组术前1 h灌胃犀角地黄汤(浓度为0.78 g/ml)0.01 ml/g,而模型组和假手术组灌胃0.01 ml/g生理盐水。造模后6 h、12 h开腹取静脉血制备血清检测丙氨酸氨基转移酶(ALT)、门冬氨酸氨基转移酶(AST)、总胆红素(TBi L)、肿瘤坏死因子α(TNF-α)和白细胞介素6(IL-6)水平;取肝组织制备匀浆检测一氧化氮(NO)、诱导型一氧化氮合酶(i NOS)、超氧化物歧化酶(SOD)和丙二醛(MDA)水平。光镜下观察肝组织病理切片的形态学改变。结果模型组和犀角地黄汤组大鼠在造模后6 h和12 h血清ALT、AST、TNF-α、IL-6水平均显著高于假手术组(P<0.01);犀角地黄汤组血清ALT、AST、TNF-α、IL-6水平均显著低于模型组(P<0.01)。各组TBi L浓度在不同时点比较差异均无统计学意义(P>0.05)。模型组和犀角地黄汤组大鼠在造模后6 h和12 h的NO、i NOS水平均显著高于假手术组,SOD水平均显著低于假手术组(P<0.01);犀角地黄汤组6 h、12 h的NO、i NOS水平均低于同时间点模型组(P<0.05)。犀角地黄汤组造模后12 h肝组织SOD水平显著高于模型组,MDA水平低于模型组(P<0.05或P<0.01)。光镜下犀角地黄汤组肝组织的肿胀、坏死、炎症细胞浸润等病理改变均较模型组轻。结论犀角地黄汤能改善脓毒症大鼠肝功能指标,减轻肝组织病理损伤,其机制可能是通过下调炎症细胞因子表达、抗氧化应激和抑制脂质过氧化途径来实现。 Objective To observe the protective function of Xijiao Dihuang Tang( 犀角地黄汤) on liver of rats with sepsis and to explore and discuss its possible mechanism. Methods Forty-eight Sprague Dawley rats were randomly divided into the sham-operated group,the model group and the Xijiao Dihuang Tang group,with 16 rats in each. Rats in the model group and the Xijiao Dihuang Tang group adopted cecal ligation and puncture to make sepsis model. The Xijiao Dihuang Tang group was given 0. 01 ml / g of Xijiao Dihuang Tang( with concentration of 0. 78 g /ml) for gavage 1 h before operation,while the model group and the sham-operated group were given saline( 0. 01 ml /g) for gavage. Vein blood was taken by laparotomy to make serum for examining levels of alanine aminotransferase( ALT),aspartate aminotransferase( AST),total bilirubinand( TBi L),tumor necrosis factorα( TNF-α),and interleukin6( IL-6) 6 h and 12 h after modeling. Liver tissue was taken to make homogenate for examining levels of nitric oxide( NO),inducible nitric oxide synthase( i NOS),superoxide dismutase( SOD),and malondialdehyde( MDA).Morphological changes in pathological sections of liver tissue were observed with light microscope. Results Six hours and 12 hours after modeling,serum levels of ALT,AST,TNF-α and IL-6 of rats in the model group and the Xijiao Dihuang Tang group were all remarkably higher than that of the sham-operated group( P〈0. 01),and serum levels of ALT,AST,TNF-α and IL-6 of rats in the Xijiao Dihuang Tang group were all remarkably lower than that of the model group( P〈0. 01). There was no statistically significant difference when comparing TBil concentration of each group at different time point( P〈0. 05). Levels of NO and i NOS of rats in the model group and the Xijiao Dihuang Tang group were all remarkably higher than that in the sham-operated group 6 h and 12 h after modeling( P〈0. 01),while SOD level was remarkably lower than that in the sham-operated group( P〉0. 01); Levels of NO and i NOS in the Xijiao Dihuang Tang group at the 6 h and 12 h were all lower than that in the model group at the same time point( P〈0. 05). SOD level in liver tissue in the Xijiao Dihuang Tang group 12 h after modeling was remarkably higher than that in the model group,while MDA level was lower than that in the model group( P〈0. 05 or P〈0. 01). Pathological changes of swelling,necrosis and inflammatory cell infiltration of liver tissues of the Xijiao Dihuang Tang group shown in Light microscope were slighter than that of the model group. Conclusion Xijiao Dihuang Tang could improve liver function indexes of rats with sepsis,and decrease pathological damage of liver tissue. Its function may be acted by decreasing the expression of inflammatory cytokine,antioxidant stress and inhibiting lipid peroxidation to achieve.
