摘要
目的观察针刺辅助抗抑郁药物治疗青少年抑郁症的临床疗效。方法 46例青少年抑郁症患者随机分为治疗组和对照组各23例,对照组仅口服抗抑郁药物,治疗组在对照组基础上给予针刺治疗,隔日治疗1次,15次为1个疗程,共治疗3个疗程。两组于治疗前后及随访3、6个月时比较汉密尔顿抑郁量表(HAMD)及抗抑郁药副反应量表(SERS)评分,并判定临床疗效及抗抑郁类药物剂量疗效。结果两组患者临床疗效比较:治疗组痊愈率26.09%,总有效率86.96%,对照组分别为13.04%及78.26%,治疗组痊愈率和总有效率均明显优于对照组(P<0.05)。两组患者抗抑郁类药物剂量疗效比较:治疗组显效率43.48%,总有效率82.61%;对照组分别为13.04%及43.48%,治疗组总有效率和显效率均明显优于对照组(P<0.05)。治疗组HAMD及SERS评分治疗后、随访3个月及6个月时较治疗前明显降低(P<0.05)。对照组治疗后、随访3个月与治疗前比较HAMD评分明显降低(P<0.05);治疗后与随访6个月时HAMD评分比较差异有统计学意义(P<0.05)。治疗组治疗后、随访3个月及6个月HAMD评分及SERS评分均低于同时间点对照组(P<0.05)。结论针刺可有效减轻抗抑郁药副反应,减少抗抑郁药的应用剂量,对控制青少年抑郁症远期疗效稳定。
Objective To observe the clinical curative effect of acupuncture with antidepressant in the treatment of adolescent depressive disorder. Methods Forty-six cases of adolescent depressive patients were randomly divided into the treatment group and the control group,with 23 cases in each. Both groups took antidepressant orally,and the treatment group was added with acupuncture treatment,once every other day. Fifteen times were taken as one treatment course,and the treatment group was given 3 courses of treatment in all. Scores of Hamilton Depression Scale（ HAMD） and Rating Scale for Side Effects（ SERS） of two groups were compared before and after treatment,as well as at follow-up visits 3 and 6 months later. Clinical curative effects and curative effects of antidepressant dose were evaluated. Results Comparison of curative effects in two groups： The recovery rate and total effective rate were26. 09% and 86. 96% in the treatment group,and 13. 04% and 78. 26% in the control group,respectively. The recovery rate and total effective rate in the treatment group was remarkably superior to those in the control group（ P〈0. 05）. Comparison of dose effect of antidepressant in two groups： The efficiency rate and total effective rate were43. 48% and 82. 61% in the treatment group,and 13. 04% and 43. 48% in the control group,respectively. The total effective rate and efficiency rate in the treatment group was remarkably superior to that in the control group（ P〈0. 05）. Scores of HAMD and SERS in the treatment group remarkably decreased after treatment,and at the 3rd month and 6th month follow-up visits compared with that before treatment（ P〈0. 05）. Scores of HAMD in the control group after treatment and at the 3rd month follow-up visit remarkably decreased compared with that before treatment（ P〈0. 05）; There was statistically significant difference in HAMD scores between after treatment and the 6th month follow-up visit（ P〈0. 05）. Scores of HAMD and SERS in the treatment group after treatment,and the 3rd and 6th month follow-up visits were all lower than that in the control group at the same time points,respectively（ P〈0. 05）.Conclusion Acupuncture could effectively reduce the side reactions of antidepressant,decrease application dose of antidepressant,and have steady long-term curative effects on controlling adolescent depressive disorder.
