摘要
目的研究硫酸镁防治蛛网膜下腔出血(SAH)后脑血管痉挛及脑组织损伤的效果及机制。方法选择2012年5月至2014年11月来本院就诊的50例SAH患者作为研究对象,随机分为两组,每组25例。观察组给予硫酸镁治疗,对照组给予常规治疗。治疗后观察硫酸镁对防止SHA后脑血管痉挛及脑组织损伤的疗效,并分析其机制。结果观察组治疗后总有效率和格拉斯哥昏迷量表(GCS)评分分别为76.00%、(12.78±2.05)分,显著高于对照组的48.00%、(10.73±2.36)分,差异具有统计学意义(P〈0.05)。观察组治疗后脑血管痉挛(DCVS)发生率和MCA血流速度分别为4.00%、(84.91±17.50)cm/s,显著低于对照组的24.00%、(102.87±15.72)cm/s,差异具有统计学意义(P〈0.05)。结论硫酸镁防治SAH后脑血管痉挛及脑组织损伤效果显著,其具体机制与Mg2+竞争性拮抗Ca2+,抑制兴奋性氨基酸及自由基等毒性化合物有关。
Objective To study the effect and mechanism of magnesium sulfate in the prevention and treatment of cerebral angiospasm and brain tissue injury after subarachnoid hemorrhage (SAH). Methods 50 patients with SAH who were treated at our hospital from May, 2012 and November, 2014 were selected as study objects and were randomized into an observation group and a control group, 25 cases for each group. The observation group were treated with magnesium sulfate and the control group routine therapy. After the treatment, the effect of magnesium sulfate in the prevention and treatment of cerebral angiospasm and brain tissue injury after SAH were observed and its mechanism was analyzed. Results The total effective rate and Glasgow Coma Scale (GCS) score were significantly in the observation group higher than in the control group [76% vs. 48% and (12.78±2.05) vs. (10.73±2.36), P 〈 0.05]. After the treatment, the incidence of delayed cerebral vasospasm (DCVs) and MCA blood flow velocity, were significantly lower in the observation group than in the control group [4% vs. 24% and (84.91±17.50) cm/s vs. (102.87±15.72) cm/s, P 〈 0.05]. Conclusions The effect of magnesium sulfate in the prevention and treatment of cerebral angiospasm and brain tissue injury after SAH is significant. Its specific mechanism relates to the competitive antagonism of Mg2+ against Ca2+ and the inhibition of excitatory amino acids, free radicals, and other toxic compounds.
