摘要
目的探讨经皮肾镜微通道气压弹道碎石(minimally invasive percutaneous nephrolithotomy,MPCNL)和输尿管软镜钬激光碎石(Transurethral flexible ureteroscope holmium laser lithotripsy,FURL)治疗输尿管上段嵌顿性结石的临床疗效。方法回顾分析两种方法治疗输尿管上段嵌顿性结石患者的临床资料。比较两组平均手术时间、平均出血量、术后感染发热率、术后结石清除率及平均住院时间。结果 79例患者中37例行MPCNL术,42例行FURL术。MPCNL组平均手术时间较FURL组短(P<0.05),但平均住院时间较FURL组长(P<0.05);MPCNL组平均出血量较FURL多(P<0.05),术后感染发热率较FURL组低(P<0.05),术后碎石成功率较FURL组高(P<0.05)。结论输尿管上段嵌顿性结石治疗方法的选择,需要取决于结石大小,肾脏积水情况,患者意愿,医生的经验等多因素。
Objective To discuss the clinical Efficacy of Upper Ureteral Calculi between minimally invasive percutaneous nephrolithotomy and transurethral flexible ureteroscope holmium laser lithotripsy. Methods Review and analyze the clinical data of Upper Ureteral Calculi of the two treatment. Compare the average time of operation,average blood loss,postoperative infection rate,clearance rate of postoperative lithiasis and average stay. Results 37 patients of total 79 were operated of MPCNL,the others were operated of FURL. Compared with the average operate time,MPCNL is shorter than FURL（ P〈0. 05）,the average stay however is longer（ P〈0. 05）. MPCNL also has more blood loss（ P〈0. 05）,higher success rates of Gravel after surgery（ P〈0. 05）and lower postoperative infection rates（ P〈0. 05）. Conclusion The selection of the treatment of Upper Ureteral Calculi depends on multiple element such as the size of the calculi,the extend of uronephrosis,patient＇s preference and the experience of the doctor.
