摘要
目的探讨输尿管逆行置管处理妊娠期上尿路结石合并感染的有效性与安全性。方法整群选择2012年7月—2015年7月该院收治的妊娠期上尿路结石合并感染患者35例,均于输尿管镜下逆行置双J管处理,对其临床资料进行回顾性分析。结果 35例患者肾绞痛、发热等急症症状均获得有效改善,体温1～3 d恢复正常,白细胞计数1周内恢复正常,孕妇胎儿稳定,均未见输尿管穿孔及其他并发症,其中2例于插管时出现感染性休克前兆,遂转ICU行抗感染、抗休克治疗1～2 d,稳定后再行输尿管镜下逆行置双J管处理,均顺利生产、婴儿健康。结论输尿管逆行置管处理妊娠期上尿路结石合并感染,可有效缓解病情、控制感染,为顺产及母婴健康提供保障,具有良好的有效性与安全性。
Objective To discuss the effectiveness and security of retrograde ureteral catheter in treatment of upper urinary tract calculi in pregnant woman combined with infection. Methods 35 cases of patients with upper urinary tract calculi in pregnant woman combined with infection admitted and treated in our hospital from July 2012 to July 2015 were selected and treated with retrograde double J tube under ureteroscope, and their clinical data were retrospectively analyzed. Results The acute symptoms such as renal colic and fever in 35 cases of patients were effectively improved, the body temperature returned to normal on 1～3d, white blood cell count returned to normal in 1 week, the pregnant women and fetuses were stable, and there was no ureteral fenestration and other complications, among them, infectious shock precursor occurred to 2cases at the time of intubation, then these 2 cases transferred ICU for anti-infection and anti-shock treatment for 1～2d, after stabilization, they were treated with retrograde double J tube under ureteroscope and then gave birth normally with healthy infants. Conclusion Retrograde ureteral catheter in treatment of upper urinary tract calculi in pregnant woman combined with infection can effectively relieve the disease condition and control infection, and provide guarantee for the natural birth and maternal and infant health, and it has good effectiveness and security.
