摘要
目的探讨丙种球蛋白及重组人干扰素α-1b分别治疗小儿手足口病(HFMD)合并病毒性脑炎的临床效果。方法选取诊治的HFMD合并病毒性脑炎患儿126例,将其随机分为球蛋白组(丙种球蛋白)和干扰素组、对照组,各42例,均给予常规护理和治疗,球蛋白组于此基础上静脉滴注丙种球蛋白;干扰素组肌注重组人干扰素α-1b。观察3组患儿的临床疗效及病情改善状况。结果与对照组相比,球蛋白组及干扰素组的有效率较高,差异具有统计学意义(χ2=7.32、3.977,P<0.05);球蛋白组有效率略高于干扰素组,差异不具有统计学意义(χ2=0.718,P=0.396);在病情改善方面上比较,3组间差异具有统计学意义(P<0.05);两两比较显示,球蛋白组和干扰素组间差异无统计学意义;球蛋白组及干扰素组各项指标时间短于对照组,差异具有统计学意义(P<0.05)。结论应用丙种球蛋白与干扰素对HFMD合并病毒性脑炎进行治疗,可明显改善预后,缩短病程,缓解患儿临床症状。
Objective To analysis the efficacy of Interferon and gamma globulin in treatment of viral encephalitis caused by hand-foot-mouth Disease(HFMD). Method 126 cases of children with viral encephalitis caused by hand-foot-mouth diseasewere randomly divided intoControl group, Globulin (IVIG) group and Interferon group, 42 cases in eachgroup. All patients were given conventional nursing and treatment. Globulin group were intravenousgamma globulin for 5 days, while Interferon group received interferon α-1 b for 3 days. The clinical curative effect and in improvement the situation of 3 groups were compared. Results Compared with the control group, the effective rate in the Globulin group and Interferon group was higher (X2 = 7.32, 3.977, P 〈 0.05 ) ; the effective rate in the Globulin group was slightly higher than that of the interferon group, the difference was not statistically significant (2,2 = 0. 718, P = 0. 396) ; while compared in the rash subsided time, the duration of fever, nervous system pathological syndrome clearance time and recovery time of cerebrospinal fluid WBC, the differences among the 3 groups was statistically significant ( F = 6. 245, 8.736, 26. 196, 6. 868, P 〈 0. 05) ;the each index of Globulin group and Interferon group was shorter than the control group, with significant difference ( P 〈 0. 05 ) ; the difference was not statistically significant between Globulin group and Interferon. Conclusions Gamma globulin and interferon in treatment of HFMD with viral encephalitis can significantly improve the prognosis, shorten the course of disease, alleviate clinical symptoms in children.
