Effect of different size flexible ureteroscopes on intrarenal pressure

摘要 目的:研究不同大小输尿管软镜对肾内压的影响。方法:在10例带有肾造瘘管患者中,使用F12/14输尿管软镜通道鞘和150cmH_2O灌注压,监测4种不同型号输尿管软镜(Olympus URF-P5、Olympus URF-V、Karl Storz Flex-X^2、Karl Storz Flex-Xc)肾内压,并记录其结果。结果:Olympus URF-P5、Olympus URF-V、Karl Storz Flex-X^2、Karl Storz Flex-Xc四种软镜的肾内压分别为(19.02±2.995)cmH_2O、(27.93±4.401)cm H_2O、(20.62±3.763)cmH_2O及(19.68±3.293)cmH_2O。Olympus URF-V肾内压显著高于其他3种软镜(P<0.01),而其他3种软镜之间的肾内压差异无统计学意义(P>0.05)。结论:相同口径输尿管软镜通道鞘,输尿管软镜的大小是影响肾内压的一个重要因素,使用直径较粗的输尿管软镜时应注意防止术中肾盂高压。 Objective:To investigate the effect of different size flexible ureteroscopes on intrarenal pressure.Method:F12/14 ureteral access sheath was used for ten patients with nephrostomy tube,and the irrigation pressure was 150cmH_2O.Intrarenal pressure in response to four different types of flexible ureteroscopes(Olympus URFP5,Olympus URF-V,Karl Storz Flex-X^2,Karl Storz Flex-Xc)were measured,and the results were analyzed.Result:The average intrarenal pressure caused by Olympus URF-P5,Olympus URF-V,Karl Storz Flex-X^2 and Karl Storz Flex-Xc were(19.02±2.995)cmH_2O,(27.93±4.401)cmH2O,(20.62±3.763)cmH_2O and(19.68±3.293)cm H_2O, respectively. Flexible ureteroscope of Olympus URF-V caused higher pressure than the other three kinds of flexible ureteroscopes(P 0.01).However,no significant difference was seen among the other three kinds of flexible ureteroscopes(P 0.05).Conclusion:When the same size ureteral access sheaths are used,the terminal size of flexible ureteroscope is an important factor influencing intrarenal pressure.It is necessary to avoid high intrarenal pressure when a thick flexible ureteroscope is used.
