摘要
目的观察生脉注射液对冠脉结扎后心肌梗死大鼠血流动力学的影响,分析生脉注射液对急性心肌梗死(AMI)后缺血心肌的保护作用。方法将SD大鼠随机分为模型组,阳性对照组(丹参注射液),生脉注射液大剂量组(12.6ml/kg)、中剂量组(6.3ml/kg)、小剂量组(2.52ml/kg),共5组。结扎左冠状动脉左前降支,造模成功后给药,并于给药后10、20、30、60min分别记录心率(HR)、收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP)、左室收缩压(LVSP)、左室舒张末压(LVEDP)和左室内压最大上升和下降速率(±dp/dtmax)。结果冠脉结扎后,各组心率(HR)减慢,SBP、DBP、MAP、LVSP和±dp/dtmax下降,LVEDP升高,其中模型组LVSP和±dp/dtmax等指标出现持续下降趋势;与模型组相比,生脉注射液各剂量组均不同程度进一步持续减慢缺血心脏的HR、降低DBP和MAP,但对SBP、LVSP、±dp/dtmax表现为先下降后回升态势,并降低缺血后LVEDP升高幅度;阳性对照组也呈现相似作用。结论生脉注射液通过减慢心率,增强心肌收缩力,降低外周阻力、缺血后LVEDP升高幅度、舒张压,并使SBP、LVSP及±dp/dtmax先降低后回升而明显改善心肌梗死大鼠心功能,由此对大鼠AMI后缺血心肌具有治疗作用,并可能在心肌缺血的早期就开始对抗心室重构引起的血流动力学变化。
Objective To observe the effect of Shengmai injection on hemodynamic in rats with myocardial ischemia, and analyse the protective effect of Shengmal injection on ischemic myocardium after AMI. Methods SD rats were randomly divided into 5 groups of model group, positive control group (Salvia mihiorrhiza injection), Shengmal injection of large dose group (12.6ml/kg), middle dose group (6.3m]/kg), small dose group (2.52ml/kg). AMI was induced by ligaturing the left ante- rior descending branch of the left coronary artery, and administrated after ligation. Then recorded the heart rate (HR), systolic blood pressure (SBP), diastolic blood pressure (DBP), mean artery pressure (MAP),left ventricular systolic pressure (LVSP), left ventricular end diastolic pressure (LVEDP), the maximum rising and dropping rate of left ventricular pressure (+dp/dtmax) in 10, 20, 30, 60rain respectively after administration. Results After coronary artery ligation, HR, SBP,DBP, MAP, LVSP and +dp/dtmax all decreased, and LVEDP increased in each group. Compared with the model group, Shengmai injection every dose group's HR significantly slowed down, DBP and MAP reduced, but SBP, LVSP and +dp/dtmax decreased at first, then recoveried, and the elevated rang of LVEDP reduced. Positive control group also showed similar effect. Conclusion By slowing down the heart rate, enhancing myocardial contractility, reducing the peripheral resistance, the elevated range of LVEDP and DBP after ischemia significantly, and recovering SBP, LVSP, ±dp/dtmax after decreases, Shengrnai injection im- prove heart function in myocardial infarction rats, and has therapeutic effect on AMI rats, meanwhile which may resist the hemodynamic changes caused by ventficular remodeling in the early stage of AMI.
