摘要
目的评价中药辨证论治对幽门螺旋杆菌相关性功能性消化不良补救治疗的有效性与安全性。方法确诊为功能性消化不良并经14C-呼气试验证实为幽门螺旋杆菌阳性患者80例,每例患者既往至少行幽门螺旋杆菌根除治疗1次以上,所有患者随机分成两组,每组40例,其中一组予以西药标准四联疗法(奥美拉唑、克拉霉素、阿莫西林及胶体果胶铋)14d。另一组结合病症予以清热解毒健脾方(安胃疡胶囊联合中药黄连、蒲公英、金银花、白花蛇舌草、枳壳、厚朴、白术、党参、黄芪、炙甘草)并联合标准四联疗法治疗14d。治疗2周评价各组症状学积分,并观察不良反应,停药1个月后复查14C-呼气试验,比较两组幽门螺旋杆菌的根除率。结果联合清热解毒健脾方组患者症状学积分明显改善,无不良反应发生,其与单纯西药组Hp根除率分别为44.7%、70.0%,幽门螺旋杆菌根除率差异有统计学意义(P<0.05)。结论联合清热解毒健脾方用于治疗幽门螺旋杆菌相关性功能性消化不良有较好的改善临床症状的疗效,并且是幽门螺旋杆菌补救治疗的重要措施之一。
Objective To explore the validity and safety of syndrome differentiation and treatment to patients with functional dyspepsia associated with helicobacter pylori eradication failure. Methods 80 patients with functional dyspepsia associated with Hp positive confirmed by ^14C-breath test were enrolled, each patient received helicobacter pylori eradication therapy at least more than one time. All patients were randomly divided into two groups, one group of 40 cases with standard quadruple therapy（Omeprazole, clarithromycin, amoxicillin and colloidal bismuth pectin） for 14 d. Another group of 40 cases was given Traditional Chinese medicine combined with quadruple therapy for 14 d. After 2 weeks of treatment, the symptoms of each group were evaluated and the adverse reactions were observed. 1 month after drug withdrawal all patients were tested again by ^14C-breath test to compare the eradication rate of H.pylori between the two groups. Results The symptom scores of the patients in the group combined with Traditional Chinese medicine were improved obviously and no adverse reactions occurred. The Hp eradication rate of the two group were 44.7% and 70.0% respectively. The difference between them was statistically significant（P〈0.05）. Conclusion The treatment of combined traditional Chinese medicine has better effect on improving clinical symptoms to patients with functional dyspepsia associated and Hp infection, which will be a promising remedial treatment for Helicobacter pylori.
