摘要
目的:探讨自然杀伤细胞(NK细胞)不同浸润程度与食管癌预后的关系。方法:将本院92例食管癌组织进行CD57染色,对NK细胞进行计数,将不同NK细胞浸润程度样本分为3组:轻度浸润组(NK细胞≤50个)30例,中度浸润组(50个<NK细胞<150个)35例,高度浸润组(NK细胞≥150个)27例。对3组患者的临床资料数据进行分析,同时采用Cox比例风险回归模型对影响预后的因素进行统计学处理。结果:(1)3组5年生存率差异有统计学意义(P<0.05),随着NK细胞浸润程度的加重,5年生存率逐渐升高;(2)随着TNM分期的进展,食管癌患者5年生存率逐渐降低(P<0.05);在Ⅲ-Ⅳ期食管癌患者中高度浸润组患者的5年生存率高于轻度浸润组(P<0.05);(3)Cox比例风险回归模型分析显示,TNM分期、肿瘤直径、NK细胞浸润程度以及肿瘤分化程度是影响食管癌预后的独立危险因子(P<0.05)。结论:NK细胞浸润程度越高食管癌患者预后越好,可将NK细胞表达量作为食管癌疗效的评定指标。
Objective: To investigate the relationship of different degree of NK cells infiltration with the prognosis of.esophageal cancer. Methods: 92 cases of esophageal cancer tissues in our hospital were dyed with CD57, we divided different degree of NK cells infiltrating samples into three groups : 30 cases of mild infiltration group ( NK in filtrating cells ≤50) , 35 cases of moderate infiltration group (50 〈 NK infiltrating cells 〈 150) and 27 cases of highly invasive group ( NK cells≥ 150 ). The clinical data of the three groups of patients were analyzed, meanwhile statistical processing by using the Cox proportional hazards regression model on the factors affecting the prognosis was taken. Results : ( 1 ) the 5- year survival rates of the three group were statistically significant ( P 〈 0.05 ), the 5-year survival rates gradually increased with the deepening of the NK cells infiltration degree; (2) with the progress of the TNM stages, the 5-year survival rates of esophageal cancer patients gradually reduced (P 〈 0.05 ). In patients with esophageal cancer in stage m- IV, 5- year survival rate of patients in the highly invasive group was higher than that in the mild group ( P 〈 0.05 ) ; ( 3 ) the Cox proportional hazards regression model analysis showed : TNM stages, diameter of tumor, NK cells infiltration degree and tumor differentiation degree were independent risk factors for prognosis of esophageal cancer ( P 〈 0.05 ). Conclusion: The higher infiltration degree of NK cells is, the better the prognosis of patients with esophageal cancer is, the expression of NK cells can be evaluation index of the esophageal cancer curative effect.
