摘要
目的:通过对隐睾患者检测神经细胞粘附分子(NCAM)及转录因子Oct-4蛋白,探讨隐睾症与生殖细胞肿瘤的关联性。方法:搜集入住我院的隐睾症的患者,采集隐睾睾丸标本,并进行睾丸活检,检测NCAM、Oct-4的表达;并分析两个指标的表达与隐睾位置和年龄的相关性。结果:NCAM在人隐睾中的表达与年龄组、睾丸位置无相关性(P>0.05)。但是大于5岁后隐睾内生精小管NCAM阳性表达有增高趋势。不同年龄组NCAM表达存在显著差异性(P<0.05),小于1.6岁年龄组与大于5岁年龄组间存在显著差异性(P<0.017),余年龄组间无显著差异性。NCAM在间质内阴性表达与睾丸位置分布无相关性(P>0.05)。Oct-4与NCAM生精小管内同时阳性表达有4例,其年龄及睾丸位置分布无规律。结论:NCAM、Oct-4在人隐睾中存在病理性表达,NCAM和Oct-4可作为临床预测隐睾患儿生殖潜能和睾丸癌变的筛查指标。
Objectives：To discuss the relevance between cryptorchidism and germ cell tumors through detecting the neural cell adhesion molecule（ NCAM） and the transcription factor Oct-4 protein in cryptorchid patients.Methods：The testicular specimens of cryptorchidism patients admitted into our hospital were collected and testicular biopsy was performed to detect NCAM Oct-4 expression.The relevance between expression of the two indicators and cryptorchidism location and age was explored.Results：1.NCAM expression in human cryptorchidism was not related to age group and the location of the testes（ P〉 0.05）.However,the NCAM positive expression of seminiferous tubule within the cryptorchidism tended to increase after 5 years＇ old.Difference in the NCAM expression among different age groups was significant（ P〉 0.05）,especially difference between the 1.6-year-old age group and the 5-year-old age group（ P〈 0.017）,while difference among other age groups was not significant.NCAM negative expression in the stroma had no correlation with the distribution of testicular position（ P〉 0.05）.There were 4 cases having positive expression of Oct-4 and NCAM within the seminiferous tubules simultaneously with irregular age distribution and testicular position.Conclusions：NCAM and Oct-4 can serve as clinical indicators to predict the reproductive potential of children and screen testicular cancer.
