实时动态血糖监测的准确性与血糖和血糖波动的关系 被引量:3

The relationship of accuracy of real-time continuous glucose monitoring system with blood glucose and blood glucose fluctuations

摘要 目的初步探讨实时动态血糖监测(RTˉCGM)系统的准确性与血糖及血糖波动关系。方法纳入50例糖尿病患者,均应用RTˉCGM系统连续监测3 d。计算指尖血糖(FSBG)与RTˉCGM探头值的匹配率,用Pearson相关分析及多元线性回归分析分析匹配率与血糖波动参数的相关性。计算不同时间点、不同FSBG点精度的FSBG与对应探头值的相对平均偏差(RAD)和ISO标准。结果匹配率与平均血糖波动幅度(MAGE)呈线性负相关。全天、餐前、餐后、夜间探头值与相应FSBG的RAD分别为1.21%、2.50%、3.75%、2.38%(P<0.05),ISO标准分别为93.90%、95.02%、91.33%、95.04%。血糖(BG)<4.5 mmol/L,4.5≤BG<7.8 mmol/L,7.8≤BG<11.1mmol/L,11.1mmol/L≤BG探头值与相应FSBG的RAD分别为13.80%、3.14%、5.10%、6.81%(P<0.01),ISO标准分别为78.12%、97.19%、92.41%、89.50%。结论RTˉCGM准确性与血糖及血糖波动有关,血糖波动越小,准确率越高;当低血糖或高血糖时,准确性均降低。 Objective To investigate the relationship of accuracy of real-time continuous glucose monitoring system(RTCGM) with blood glucose and blood glucose fluctuations. Methods Total of 50 diabetic patients was included. Patients were monitored for 72 h using RT-CGM. The matching rate of paired values(FSBG/ RT-CGM) were counted,and the correlation of the matching rate with blood glucose fluctuations was analyzed by correlation analysis and multiple linear regression analysis. Furthermore,we calculated RAD,and ISO criteria of paired values(RT-CGM/FSBG)of all-day,fasting,postprandial,and nighttime. Results The matching rate was negatively correlated with the mean amplitucle of glycaemic excursion(MAGE). Of all day,fasting,postprandial and nighttime,the RAD were 1.21%,2.50%,3.75%,and 2.38%(P0.05),respectively, and the ISO criteria were 93.90%,95.02%,91.33%,and 95.04%,respectively. For BG4.5 mmol/L,4.5≤BG7.8 mmol/L,7.8≤BG11.1 mmol/L,11.1 mmol/L≤BG,the RAD were 13.80%,3.14%,5.10%,6.81%,respectively(P0.01),and the ISO criteria were 78.12%,97.19%,92.41%,89.50%,respectively. Conclusions Blood glucose fluctuations influenced the accuracy of RT-CGM sensor values,the smaller of the blood glucose fluctuations,the higher of the accuracy. Accuracy was reduced at hypoglycemia or hyperglycemia.
