摘要
目的探讨低分子肝素钠抗凝治疗对D-二聚体升高的慢性阻塞性肺疾病患者凝血及血气分析指标的影响。方法将90例D-二聚体升高的急性加重期慢性阻塞性肺疾病患者随机分成观察组及对照组,每组45例,2组患者均采取常规对症综合治疗,在此基础上,观察组给予低分子肝素钠注射液皮下注射,4 000 IU/次,每日1次,2组疗程均为10 d,观察2组患者治疗前后肺功能、D-二聚体、凝血指标及血气分析变化。结果 2组患者治疗后肺功能指标均较治疗前显著改善(P均<0.05),观察组肺功能指标改善程度均明显优于对照组(P均<0.05);2组患者治疗后D-二聚体含量均较治疗前显著下降(P均<0.05),观察组D-二聚体含量显著低于对照组(P<0.05);对照组治疗前后凝血指标比较差异均无统计学意义(P均>0.05),观察组治疗后凝血指标较同组治疗前及对照组治疗后明显改善(P均<0.05);2组患者治疗后血气分析指标均较治疗前明显改善(P均<0.05),观察组治疗后血气分析指标均较对照组明显改善(P均<0.05)。结论低分子肝素钠抗凝治疗D-二聚体升高的慢性阻塞性肺疾病患者,能够有效改善肺功能,降低D-二聚体,改善凝血状态及呼吸功能,值得临床推广应用。
Objective It is to explore the effect of low-molecular-weight heparin sodium in anticoagulant therapy on coagulation indicator and blood gas analysis of patients suffering from chronic obstructive pulmonary disease ( COPD with increasing of D - dimer. Methods 90 cases of patients suffering from COPD in acute exacerbation with increasing of D - dimer were randomly divided into the observation group and the control group, each group had 45 cases. All the patients in both groups were treated with conventional symptomatic composite therapy. On this basis, the observation group was given low-molecular-weight heparin sodium injection by hypodermic injection, 5 000 IU each time, one time a day. Both groups were treated for ten days. The change of lung function, D - dimer, coagulation indicator, and blood gas analysis between the two groups before and after treatment were observed. Results Both groups had a significant improvement on lung function index after treatment (P 〈 0.05 ) , and the extent of improvement of the observation group was more significant than the control group ( P 〈 0.05 ) ; Both groups had a significant reduction on the D - dimer content after treatment (P 〈 0. 05) , and the D - dimer content of the observation group was significantly lower than the control group (P 〈 0. 05) ; The coagulation indicator of the control group had no statistical difference between before and after treatment (P 〉 O. 05) , and the coagulation indicator of the observation group had a obvious improvement than before treatment within the group and after treatment of the control group ( P 〈 O. 05 ) ; Both groups had an obvious improvement on blood gas analysis indexes after treatment (P 〈0. 05) , and the observation group after treatment had more obvious improvement on the blood gas analysis index than the control group (P 〈 0.05). Conclusion For patients suffering from COPD with increasing of D -dimer, low-molecular-weight heparin sodium in anticoagulant therapy can effectively improve lung function, reduce D -dimer, and improve coagulation state and respiratory function, which is worthy of clinical expansion and application.
