摘要
目的探讨对高血压患者进行社区综合干预的疗效。方法选择2012年6月至2014年6月北京市西城区广内医院收治的高血压患者300例,采用随机数字表法分为对照组和干预组各150例。两组均给予厄贝沙坦和氢氯噻嗪治疗,而干预组在此基础上增加综合干预,主要包括定期宣传教育、建立个人档案、改变生活习惯、坚持运动锻炼和积极开展交流等。观察比较两组的高血压知晓率、治疗依从性和控制率,以及生活方式。结果干预组患者的高血压知晓率、按时服药率和血压控制率显著高于对照组;治疗1年后干预组的平均收缩压和舒张压、抽烟酗酒、摄取高盐高糖高脂饮食、缺乏锻炼以及不良情绪等发生率均显著低于对照组,差异均有统计学意义(P<0.05)。结论在基础治疗基础上,对社区高血压患者采取综合干预措施能提高降压效果,且生活方式显著改善,值得社区推广。
Objective To investigate the effects of a comprehensive community intervention therapy for patients with high blood pressure. Methods Three hundred patients with hypertension in communities from Jun 2012 to Jun 2014 patients were randomly divided into the control and the intervention groups, 150 cases in each group. The control group was treated with regular medication by using irbesartan and hydrochlorothiazide, while the comprehensive community intervention therapy was added to the intervention group. The intervention included the periodic public education, creation of healthy profile, changing the habits, insisting the exercise, and conducting the active exchanges. The hypertension awareness, compliance and control rates, and lifestyle after treatment were compared between the two groups. Results The hypertension awareness rate, taking medication on time rate and blood pressure control rate in the intervention group were 86% ,94% and 79. 3%, respectively. These were significantly higher than those in the control group （74%, 84. 7% and 66. 0% ,respectively）. After 1 year of treatment,the mean systolic and diastolic blood pressure in the intervention group was significantly lower than that in the control group. The rates of smoking and drinking, high-sah / high-sugar / high-fat intake,lack of exercise and bad mood in intervention group were of 8.7 % ,34%, 9. 3 % and 2%, respectively. These were also significantly lower than those in the control group. Conclusion The comprehensive interventions for patients with hypertension in communities have a good effect. The lifestyle of the patients is significantly improved. It is worth for community promotion.
