摘要
目的探讨胃癌术后乳腺癌易感基因1(breast cancer susceptibility gene 1,BRCA1)及X射线修复交叉互补基因1(X-ray repair cross complementing gene 1,XRCC1)表达及其奥沙利铂辅助化疗效果。方法选取74例术后病理诊断为胃癌患者,均予奥沙利铂辅助化疗。采用免疫组化法检测病理标本中BRCA1和XRCC1的蛋白表达情况。结果 74例患者胃癌组织内XRCC1及BRCA1蛋白表达的阳性率分别为18.9%(14/74)、29.7%(22/74);BRCA1与XRCC1表达呈正相关(r=0.264,P=0.012);BRCA1及XRCC1蛋白表达阳性患者化疗有效率以及3年无病生存率(diseasefree survival,DFS)和总生存期率(overall survival,OS)均低于蛋白表达阴性患者,差异均有统计学意义(P<0.05)。BRCA1和XRCC1蛋白的表达及肿瘤分化程度是胃癌患者术后化疗3年OS的独立因素。结论胃癌患者BRCA1和XRCC1蛋白的表达是影响患者预后的关键因素,且影响术后辅助化疗的治疗效果。临床上,可根据其表达水平选取化疗药物。
Objective To explore the correlation between expressions of breast cancer susceptibility gene 1 （BRCA1） and X- ray repair cross complementing gene 1 （ XRCC1 ） and effect of oxaliplatin adjuvant chemotherapy. Methods Seventy four postoperative patients who were pathologically diagnosed as gastric cancer were selected to receive the oxaliplatin adjuvant chemotherapy. Expressions of BRCA1 and XRCC1 in the cancer tissues were measured by using immunohistochemistry. Results In the 74 patients, the positive expression rates of XRCC1 and BRCA1 were 18.9% （14/74） and 29.7% （22/74） ,respectively. The expression of XRCC1 protein was positively correlated with BRCA1 protein in advanced gastric cancer （r = 0. 264 ,P = 0. 012）. The chemotherapeutic effect, rate of 3 year disease-free survival （DFS） and overall survival （OS） in the patients with positive expression of XRCC1 and BRCA1 were all lower than that in patients with negative expression of the two genes （ P 〈 0. 05 ）. COX multivariate analysis showed that the status of XRCC1 and BRCA1 proteins expressions and differentiation degree were independent factors for 3 year OS in these patients. Conclusion The expressions of XRCC1 and BRCA1 protein in postoperative patients with gastric cancer tissues are a key factor affecting the prognosis of the patients, and also affect the treatment effect of adjuvant chemotherapy. Therefore, chemotherapy drugs can be selected according to their expression levels.
