显微内镜腰椎间盘摘除术治疗腰椎间盘突出的效果及预后被引量：8

摘要目的比较显微内镜腰椎间盘摘除术（MED）以及传统常规开窗手术治疗腰椎间盘突出（LDH）的效果及影响预后的相关因素。方法回顾分析98例行显微内镜腰椎间盘摘除技术治疗LDH患者的临床资料,对照组60例行传统常规开窗手术治疗。按照Macnab标准评定两组治疗效果,并分析影响MED治疗LDH预后的相关因素。结果 MED组术后平均4 d下地活动,平均住院时间10.5 d;平均15.3 d生活自理,6 w恢复工作。随访3~12个月后,MED组治疗率显著高于对照组（χ~2=22.768,P〈0.05）。MED组并发症显著低于对照组（χ~2=5.815,P〈0.05）。临床评分结果显示,MED组ODI及JOA评分均显著优于对照组（均P〈0.05）;MED组患者术中出血量、病程、多节段病变、恐惧及焦虑心理、术后功能锻炼与对照组有统计学差异（均P〈0.05）;进一步多因素非条件Logistic回归分析显示,术中出血量、多节段病变、术后功能锻炼是影响MED组预后的独立危险因素。结论 MED术治疗LDH术后恢复快、效果较好,术中出血量少、住院时间短、无多节段病变及坚持术后功能锻炼是影响其治疗预后的主要危险因素。
