摘要
目的探讨血清中Obestatin对急性胰腺炎严重程度的鉴别价值。方法选取2013年1月至2015年3月我科收治的60例急性胰腺炎患者为观察组,其中轻症急性胰腺炎(mild acute pancreatitis,MAP)患者30例,重症急性胰腺炎(severe acute pancreatitis,SAP)患者30例。对照组30例为同期健康体检者。采用酶联免疫吸附试验(ELISA)测定患者入院第1天和治疗后1周血清中Obestatin水平,同时采用放射免疫分析法(RIA)测定血清IL-1β、TNF-α,硫代巴比妥酸(TBA)法测定血清丙二醛(MDA),黄嘌呤氧化酶法测定血清超氧化物歧化酶(SOD)水平。结果 MAP组和SAP组患者入院第1天Obestatin、IL-1β、TNF-α和MDA水平明显高于对照组(P<0.05),且SAP组患者Obestatin、IL-1β、TNF-α和MDA水平高于MAP组(P<0.05),MAP组和SAP组患者SOD水平低于对照组(P<0.05);治疗1周后MAP组和SAP组患者Obestatin、IL-1β、TNF-α和MDA水平明显下降,但仍然高于对照组(P<0.05),SOD水平上升,仍然低于对照组(P<0.05)。结论血清Obestatin升高与急性胰腺炎严重程度有关,Obestatin水平高低可能与炎症因子和氧化应激水平相关,Obestatin可作为判断急性胰腺炎严重程度的指标之一。
Objectlve To explore the diagnostic value of serum Obestatin in discriminating the severity of acute pancreatitis. Methods Sixty acute pancreatitis patients including 30 patients with mild acute pancreatitis ( MAP group) and 30 patients with severe acute pancreatitis (SAP group), and 30 cases of healthy controls were involved in this study. The serum levels of Obestatin in patients at admission one day and after one weeks treatment and health controls were measured by ELISA, the serum levels of IL- 1 β and TNF- α were detected by RIA, and TBA was used to test the serum level of MDA and xanthine oxidase was used to measure the serum level of SOD. Results Compared with control group, the serum levels of Obestatin, IL-1β, TNF-α and MDA at admission one day were increased in MAP and SAP groups (P 〈 0.05 ) , and moreover the serum levels of Obestatin, IL- 1 β, TNF- α and MDA in SAP group were higher than that of MAP group ( P 〈 0.05 ), and the serum level of SOD in MAP and SAP groups was less than control group ( P 〈 0.05 ). The serum levels of Obestatin, IL-1 β, TNF-α and MDA were decreased in MAP and SAP groups after one weeks treatment, but still higher than that of control group ( P 〈 0.05 ), and the serum level of SOD was increased, still lower than that of control group ( P 〈 0.05 ). Conclusion The increase of Obestatin level in serum was associated with the severity of acute pancreatitis, and the serum level of Obestatin may be correlated with inflammatory cytokines and oxidative stress,which can be used to predict the severity of acute pancreatitis.
