儿童鼻腔纽扣电池异物嵌顿临床特征分析 被引量:3

摘要 目的分析儿童鼻腔纽扣电池鼻腔异物嵌顿临床特征并探讨不同取出异物方式的预后及其治疗策略。方法回顾性分析2009年1月至2014年1月46例纽扣电池鼻腔异物患儿的临床资料,14例在无麻醉传统前鼻镜常规方式下取出(常规组),32例在全身麻醉鼻内窥镜检查下取出(鼻内窥镜组)。取出鼻腔组扣电池后,所有患儿均经鼻腔冲洗治疗并随访6个月。结果46例患儿鼻腔黏膜均存在腐蚀性损伤,常规组与鼻内窥镜组患儿鼻中隔穿孔率差异无统计学意义(X2=0.047,P〉0.05),但鼻内窥镜组患儿鼻腔粘连恢复快于常规组,差异有统计学意义(x2=4.43,P〈0.05)。结论鼻内窥镜探查下纽扣电池鼻腔异物取出有助于明确鼻腔黏膜损伤范围与损伤程度,同时有助于术中充分的鼻腔创面冲洗,缓解纽扣电池内容物所造成的继发性损伤,在条件充分的情况下尽量选择鼻内窥镜探查下异物取出。规范的临床随访及鼻腔冲洗有助于缓解鼻腔粘连的发生。
