4项实验室指标联合检测在冠心病早期诊断中的应用价值 被引量:22

摘要 目的探讨联合检测D-二聚体(D-D),纤维蛋白原(FIB),同型半胱氨酸(Hcy)及超敏C反应蛋白(hsCRP)在冠心病早期诊断中的应用价值。方法选择2014年1月至2015年5月该院心内科收治的272例冠心病患者为疾病组,根据检查结果分为3个亚组,其中稳定型心绞痛(SAP)组90例、不稳定型心绞痛(UAP)组90例和急性心肌梗死(AMI)组92例;选取同期该院健康体检人群100例作为对照组。分别测定各组对象的血浆D-D、FIB水平,血清Hcy及全血hs-CRP水平。结果 SAP组、UAP组、AMI组的D-D、FIB、Hcy及hs-CRP的水平均高于对照组,差异有统计学意义(P<0.01)。AMI组心功能Killip分级比较,随着Killip分级的增加,AMI患者D-D、FIB、Hcy及hs-CRP水平亦逐渐升高,其中D-D、FIB和Hcy水平在Ⅰ级和Ⅲ级、Ⅱ级和Ⅲ级之间差异均有统计学意义(P<0.05),但是hs-CRP在各级之间差异无统计学意义(P>0.05)。联合检测D-D、FIB、Hcy及hs-CRP 4项指标可提高冠心病早期诊断的阳性率。结论 D-D、FIB、Hcy及hs-CRP 4个实验室指标与冠心病密切相关,联合检测该4项指标有助于冠心病患者的早期诊断。
