摘要
目的:观察肝硬化失代偿期间静脉系统和心功能的改变,心功能的变化与门静脉系统变化是否有相关性。方法:采用2.5MHz、3.5MHz两种探头分别测量30例肝硬化和32例正常人心脏及门静脉各项指标。心脏测量分别取左室长轴切面,心尖四腔心切面,主动脉短轴切面。门静脉测量选右助间斜切面.结果:肝硬化组患者门静脉内径(D)明显增宽(P<0.01),血流量(Q)明显增多(P<0.01)。肝硬化组患者右心功能中肺动脉瓣加速时间(ACT)明显缩短(P<0.01),三尖瓣舒张早期峰值速度(EV)明显降低(P<0.01),三尖瓣舒张晚期峰值速度(AV)明显增加(P<0.01);EV/AV明显降低(P<0.01)。门静脉血流量与EV、EV/AV呈负相关,与ACT、AV呈正相关。结论:肝硬化失代偿期门静脉内径增宽,血流量明显增多;心功能中右心收缩功能和舒张功能明显受累。通过测量门静脉的血流量可间接估计心功能。
Objective: To observe the changes of portal vein and cardiac function in decompensation stage of liver cirrhosis, the correlation between both. Methods: The cardiac function changes and portal vein parameter changes of 30 patients with liver crisis in decompensation stage compared with 32 health persons. Results:The diameter of portal vein widened (P<0. 01) and blood volume obviously increased in patients with liver cirrhosis (P<0. 01) .In rightcardiac function ACT shortened (P<0. 01) , the EV of tricuspid valve decreased (P<0. 01) , the AV of tricuspid valve increased (P<0. 01), EV/AV decreased markedly (P<0. 01). The correlation among blood volume of portal vein and EV. EV/AV was negative. Conclusion: In decompensation stage of liver cirrhosis, the diameter of portal vein widen and blood volume increase obviously; the right ventricular systole and diastole function are affected greatly, the wider the portal vein diameter is. the more the blood volume is. These directly affect the cardiac functions.
