摘要
目的:探讨脑血流单光子断层显像(rCBF-SPECT)及磁共振血管造影(MRA)对椎基底动脉供血不足(VBI)性眩晕的评价价值。方法:对121例VBI患者(VBI组)和41例前庭周围性疾病(PVD)患者(对照组)均行rCBF-SPECT显像检查;VBI组66例,对照组29例行MRA检查。结果:VBI组rCBF-SPECT异常率93.38%.多数发生在左颞,MRA异常率65.15%(43/66),两者与对照组之间的差异均有显著性(P<0.01);VBI组rCBF-SPECT异常率明显高于MRA(P<0.01)。结论:rCBF-SPECT有助于脑功能及脑血流灌注状态的研究,MRA有助于脑供血减少的病因学研究,两者对VBI性眩晕的诊断和治疗均有指导意义。
Objective: To explore the clinic value of rCBF-SPECT (regional cerebral blood flow-single photon emission computerized tomography ) and MRA (magnetic resonance angiography ) to evaluating dizziness because vertebrobasilar arterial insufficiency. Methods: One hundred and twenty-one patients with vertebrobasilar arterial insufficiency (VBI) and 41 patients with vestibule peripheral disease (PVD) were examined by rCBF-SPECT; 66 VBIpatients and 29 PVD patients were examined by MRA. Results: Compared with PVD patients (control group), positive rate of rCBF-SPECT (93. 38%) and positive rate of MRA (65. 15%) in VBI group significantly increased (P< 0. 01). Compared with positive rate of MRA, the positive rate of rCBF-SPECT significantly increased (P<0. 01) in VBI group. Most of disease sections detected by rCBF-SPECT were in left temple of brain. Conclusion: rCBF-SPECT detect contribute to study of regional cerebral blood flow, MRA detect contribute to determinate etiological factor of VBI, rCBF-SPECT and MRA all posses clinic value for evaluating dizziness because vertebrobasilar arterial insufficiency.
