摘要
[目的]探讨奥曲肽联合固本消胀汤治疗恶性肠梗阻的临床疗效。[方法]将33例恶性肠梗阻患者随机分为2组:治疗组16例,对照组17例。治疗组在常规治疗(即禁食,胃肠减压,抗感染,维持水电解质平衡,营养支持,止痛等)基础上,予固本消胀汤煎剂鼻饲,100ml/次,2次/d,鼻饲后夹闭胃管2h,同时予奥曲肽持续静脉泵入,25μg/h,治疗7d。对照组在常规治疗基础上予奥曲肽持续静脉泵入,25μg/h,治疗7d。[结果]治疗组完全缓解5例,部分缓解10例,无效1例,有效率93.8%。对照组完全缓解2例,部分缓解7例,无效8例,有效率52.9%。与治疗前比较,患者临床症状、腹部平片、胃肠减压引流量、KPS评分、1年生存率治疗后明显改善,差异有统计学意义(P<0.05)。[结论]奥曲肽联合固本消胀汤治疗恶性肠梗阻的临床有效率高,显示中西医联合治疗恶性肠梗阻的优势,值得临床推广。
[Objective]To investigate the clinical effect of octreotide in combination with GuBenXiaoZhang prescription for the treatment of maligant bowel obstruction.[Methods]Thirty-three patients with malignant obstruction were randomly divided into two groups：treatment groups（16cases）and control groups（17cases）.Both patients were given conventional treatment,such as fasting,gastrointestinal decompression,anti-infection,water and electrolyte balance,nutritional support,pain-killing treatments and so on.Besides common treatments,patients of treatment groups were treated with GuBenXiaoZhang prescription and octreotide for 7days.GuBenXiaoZhang prescription,100 ml each time,2times daily,clip the stomach after nasogastric tube 2h,followed with peptide continuous pumping,25μg/h Patients of control groups were given the peptide continuous intravenous pumping（25μg/h）for 7days.[Results]In treatment groups,CR（complete response）5cases,PR（partial response）10cases,NC（invalid）1case,the clinical effective rate was 93.8%（15/16）.In control groups,CR（complete response）2cases,PR（partial response）7cases,NC（invalid）8cases,the clinical effective rate was 52.9%（9/17）.Compared with prior treatments,there existed siginificantly difference in clinical symptoms,abdominal plain film,gastrointestinal decompression traffic and KPS score.At the same time,the urvival rate was obviously improved after treatment（P〈0.05）.[Conclusion]Octreotide combined with GuBenXiaoZhang prescription shows higher efficiency in the treatment of malignant obstruction.It is worth to generalize this clinical treatment,which combined westernmedicine with traditional Chinese medicine.
