摘要
作者用二维造影超声心动图(2-DCE)评价35例动脉导管未闭(PDA)病儿的左向右分流量,发现2-DCE肺动脉远端舒张期负性影象(DNCE)随PDA左向右分流量的增加而增大。对2-DCE和M型超声心动图(ME)多项指标数据进行逐步回归分析,筛选出Ⅱ级DNCE、左室后壁搏幅实测值与其正常值之比(LVPWEP)、Ⅰ级DNCE和左房径实测值与其正常值比(LADR)4项重要指标,建立了回归方程,可无创性定量诊断PDA左向右分流量。将PDA组35例和另选PDA11例病儿的指标数据代入该方程进行考核,预测正确率达80%。
The assessment of left-right ductalshunting flow in 35 children with PDA by2-DCE with 35 normal children as a controlwas shown in the study.A close relationshipwas demonstrated between the grade of thediastolic negative contrast at the distal endsof the pulmonary artery and Q p/Qs. Thediastolic negative contrast effect of 2-DCEcould be used as a new important indexin the assessment of left-to-right ductalshunting flow. Four parameters which were closely cor-related with Qp/Qs were selected from 8 par-ameters by the stepwise multilinear regressionprocedure.The 4 parameters were grade ⅡDNCE, LVPWER, grade I DNCE, LADR.A regression equation which was capable ofassessingQ p/Qs noninvasively in childrenwith PDA was derived from the 4 parame-ters.The accuracy of calculation of Qp/Qsby the regression equation was 80%.
