摘要
目的探讨血清N-末端脑钠肽(NT-pro BNP)、胱抑素C(Cys C)与慢性心力衰竭患者心功能、左室重构的相关性。方法入选慢性心衰(CHF)患者60例,根据纽约心功能分级标准分为Ⅱ、Ⅲ、Ⅳ级组。20例体检健康者作为对照组。测定血清NT-pro BNP和Cys C浓度。根据CHF患者血清胱抑素C浓度中位数,分为胱抑素C正常组(Cys C浓度<1.5 mg/L)和胱抑素C升高组(Cys C浓度>1.5 mg/L)。通过超声心动图测量相关指标并计算心室重构指标即左室质量指数(LVMI),计算左心室射血分数(LVEF)。对这些指标进行组间比较,并分析血清N-末端脑钠肽(NT-pro BNP)、血清胱抑素C与心室重构指标的相关性。结果 1 CHF患者血清NT-pro BNP、Cys C、显著高于对照组(P<0.01或P<0.05)。不同NYHA心功能分级中NT-pro BNP、Cys C、LVMI数值逐渐增高,各组间均有显著性差异(P<0.01)。2 Cys C升高组NT-pro BNP、LVEDd、LVMI较Cys C正常组高,Cys C升高组LVEF较Cys C正常组低(P<0.01)。3 Pearson相关性分析结果显示,CHF患者血清NT-pro BNP和Cys C水平与LVMI呈正相关(P<0.01);CHF患者血清NT-pro BNP水平与LVEF呈负相关(P<0.01)。结论 NT-pro BNP和Cys C水平与心力衰竭的严重程度及左室重构相关。
Objective To investigate the correlation between serum N-terminal pro-brain natriuretic peptide( NT-pro BNP),cystatin C and left ventricular remodeling in patients with chronic heart failure( CHF). Methods A total of 60 patients with CHF were divided into three groups based on the standard of New York Heart Association Classification,and 20 healthy volunteers were selected as control group. Serum NT-pro BNP and cystatin C 1evels were detected. Left ventricular mass index( LVMI) and left ventficular ejection fraction( LVEF) were calculated by echocardiography. Correlation between serum NT-pro BNP,cystatin C and left ventricular remodeling in patients with CHF were analyzed. Results Levels of NT-pro BNP、LVMI and cystatin C in patients with CHF were significantly higher than those of control group( P 0. 01 or P 0. 05). Levels of NT-pro BNP、LVMI and LVEDd in group with higherconcentrationofcystatin C were significantly higher than that of group with normalconcentrationofcystatin C( P 0. 01). Pearson correlation analysis showed that the Levels of NT-pro BNP and cystatin C were positively correlated with LVMI( P 0. 01). Conclusion Serum NT-pro BNP and cystatin C are correlated with cardiac function and left ventricular remodeling.
