摘要
目的:探讨地佐辛与异丙嗪联合用于游离腓骨瓣颌骨重建术后镇痛镇静的可行性。方法:择期行游离腓骨瓣颌骨重建术的患者60例,随机分为对照组(C)、舒芬太尼组(S)和地佐辛+异丙嗪组(DY),每组20例。术毕前1 h双盲法分别静注生理盐水、舒芬太尼0.1μg/kg、地佐辛0.1 mg/kg+异丙嗪0.5 mg/kg。手术结束时再给药一次,剂量相同。记录术毕0.5、2、6、12、24 h患者心率、平均动脉压、动脉血CO2分压、疼痛视觉模拟评分及躁动-镇静评分。结果:地佐辛+异丙嗪组的镇痛时间长于舒芬太尼组,并且在前四个时点的镇静评分也低于舒芬太尼组;后者的动脉血CO2分压在术毕0.5 h、2 h两个时点升高。结论:地佐辛与异丙嗪联合应用,有助于增强镇痛镇静作用,减轻呼吸抑制,用于游离腓骨瓣颌骨重建术后镇痛镇静是可行的。
Objective: To discuss the analgesia and sedative effect of combined medication with dezocine and promethazine on patients after jaw reconstruction with free fibula flap. Methods: Sixty patients following jaw reconstruction with free fibula flap were divided randomly into control group(C), sufentanil group(S), dezocine plus promethazine group(DY), 20 patients in each group. Physiological saline, sufentanil(0.1 μg/kg), dezocine(0.1 mg/kg) plus promethazine(0.5 mg/kg) were administered intravenously for each group 1 h before the end of the operation. At the end of the operation, we used the same dosage for each group once more. HR, MAP, Pa CO2, Visual analogue scale(VAS) and Richmond Agitation-Sedation Scale(RASS) were recorded at the time points of 0.5, 2, 6, 12, 24 h after operation. Results: The analgesia persistence time of group DY was significantly longer than that of group S and RASS of group DY was significantly lower than that of group S at the 4 time points. The Pa CO2 of group S was significantly increased in 2 hours after operation. Conclusion: It is feasible to carry out postoperative analgesia and sedation for patients following jaw reconstruction with free fibula flap by using combined medication with dezocine and promethazine.
