Application of 64-slice spiral computed tomography angiography in studying diameter and cross-sectional area of vertebral artery

摘要 【目的】探讨64层螺旋CT血管成像在研究发育正常椎动脉直径和横截面积中的应用价值。【方法】28例临床诊断或拟诊为椎基底动脉供血不足的患者(患者组)及9例健康体检者(体检组)行头颈64层螺旋CTA检查,应用容积再现、最大密度投影、曲面重建和高级血管分析等后处理技术对其进行观察,统计分析发育正常椎动脉直径和横截面积、C4横突孔直径和横截面积。【结果】对患者组及体检组双侧椎动脉直径和横截面积、双侧C4横突孔直径和面积分别进行正态性检验,患者组0.998>P>0.855,体检组0.997>P>0.812,差异均无统计学意义(P>0.05);对患者组及体检组双侧椎动脉直径和横截面积、双侧C4横突孔直径和面积分别进行方差齐性检验及两组独立样本t检验,方差齐性检验0.855>P>0.196,两组独立样本t检验0.990>P>0.668,差异均无统计学意义(P>0.05);将患者组与体检组数据合并后(合并组)对双侧椎动脉直径和横截面积、双侧C4横突孔直径和面积分别进行正态性检验,0.999>P>0.687,差异无统计学意义(P>0.05);对合并组双侧椎动脉直径和横截面积、双侧C4横突孔直径和面积分别进行多元相关分析,0.792<r<1,差异有统计学意义(P=0.000)。【结论】64层螺旋CTA能够清晰客观的显示椎动脉的管径结构,对了解椎动脉影像表现及指导临床应用有重要的临床价值。 【Objective】To evaluate the application of 64-slice spiral computed tomography angiography(CTA) in studying diameter and cross-sectional area of vertebral artery with normal development.【Methods】Head and neck 64-slice CTA was performed in 28 cases diagnosed or suspected as vertebrobasilar artery insufficiency(lesion group) and 9 healthy people(control group). The image data of vertebral artery were postprocessed by volume rendering(VR), maximum intensity projection(MIP), curved planar reformation(CPR) and advanced vessel analysis(AVA). The normal lumen diameters and cross-sectional areas of vertebral artery and transverse foramen of bilateral C4 in normal development were statistically analyzed.【Results】The diameters and cross-sectional areas of vertebral artery and transverse foramen of bilateral C4 in both groups were analyzed with test of normality, and there was no significantly difference in teo group(lesion group 0.998〉P〉0.855; control group 0.997〉P〉0.812). There was no significantly difference(P〉0.05) in the normal lumen diameters and cross-sectional areas of bilateral vertebral arteries and transverse foramen of bilateral C4 in two groups by respectively Levene's test for equality of variances(0.855〉P〉0.196) and independent samples test(0.990〉P〉0.668). There was no significantly difference(P〉0.05) in the normal lumen diameters and cross-sectional areas of bilateral vertebral arteries and transverse foramen of bilateral C4 by analyzing combination group with test of normality(0.999〉P〉0.687). There was significantly difference(P=0.000〈0.001) in the normal lumen diameters and cross-sectional areas of bilateral vertebral arteries and transverse foramen of bilateral C4 by analyzing combination group with multivariate correlation analysis(0.792〈r〈1).【Conclusion】The anatomic structure of vertebral artery can be clearly and objectively displayed by 64-slice CTA. It has a high application value in studying the CTA features of vertebral artery and directing clinical practice.
