骨代谢相关指标与骨密度改变相关性研究 被引量:4

The relationship between bone metabolism indexes and bone mineral density change in patients with osteoarthritis

摘要 目的了解骨代谢指标与骨密度改变之间关系,探索骨代谢指标与骨质疏松关系。方法测定49例患者血清骨源性碱性磷酶,骨钙素和抗酒石酸酸性磷酸酶,及股骨颈、腰椎腕部骨密度T值,并分析其相关性。结果骨钙素与抗酒石酸酸性磷酸酶在骨质疏松患者水平增高,骨钙素与腰椎骨密度(r=-0.324,P=0.041)、腕部骨密度(r=-0.368,P=0.019),抗酒石酸酸性磷酸酶与腕部骨密度(r=-0.368,P=0.019)呈负相关。骨源性碱性磷酸酶在骨质疏松组呈最低值,且与骨量改变无相关性。指标骨源性碱性磷酸酶、骨钙素与破骨细胞活性指标抗酒石酸酸性磷酸酶之间均有很好的相关性。结论骨质疏松时,骨代谢增加,骨钙素与抗酒石酸酸性磷酸酶增加,似乎腕部骨密度改变更能体现全身内分泌因素的作用。从整体来说,成骨作用与破骨作用保持一种平衡状态。 Objective To investigate the relationship between bone metabolism indexes and changes of bone mineral density and find further the relationship between bone metabolism indexes and osteoporosis. Methods 49 patients with osteoporosis were recruited and examined serum bone metabolism indexes including calcitonin, bone-specific alkaline phosphatase and tartrate resistant acid phosphatase. At the same time the bone density of femoral neck, umbar vertebral body and wrist were measured and their correlation with serum bone metabolism indexes were analyzed. Results Calcitonin and tartrate resistant acid phosphatase increased in patients with osteoporosis. There was a negative correlation between calcitonin with bone density of umbar vertebral body (r=-0.324,P=0.041) and wrist(r=-0.368, P=0.019),and tartrate resistant acid phosphatasethe with wrist bone density (r=-0.368,P=0.019).The level of bone-specific alkaline phosphatase was the lower in patients of osteoporosis and there was no correlation with bone density .We also found that there was correlation of bone-specific alkaline phosphatase, calcitonin and tartrate resistant acid phosphatase. Conclusion Bone metabolism increased in patients of osteoporosis and calcitonin and tartrate resistant acid phosphatase increased. It seemed that changes of wrist bone density can reflect the role of systemic endocrine factors. From the overall, osteogenesis effect and osteoclasts maintain a state of balance.
