血清HE4检测在妇科附件肿物诊断中的应用价值探讨

Application value of serum HE4 detection in diagnosis of gynecological adnexal masses

摘要 目的探讨血清人附睾上皮分泌蛋白4(HE4)在妇科附件肿物诊断中的应用价值。方法选取2012年1月-2015年4月该院收治的妇科附件肿物患者219例为研究对象,按照术后病理结果分为良性病变组(191例)和恶性病变组(28例),另选取同期健康体检妇女405例为对照组。采用酶联免疫吸附法检测所有研究对象血清HE4水平,并对结果进行比较分析。结果恶性病变组血清HE4水平明显高于良性病变组和对照组(P<0.05)。根据受试者操作特性曲线(ROC曲线)分析,未绝经者HE4检测附件恶性病变的临界值为61.7 pmol/L,敏感度和特异度分别为85.7%和95.3%;绝经者HE4检测附件恶性病变的临界值为72.9 pmol/L,敏感度和特异度分别为78.6%和91.2%;两者的曲线下面积分别为0.959、0.883。结论血清HE4测定是妇科良恶性附件肿物鉴别诊断的良好指标。 Objective To explore the application value of serum human epididymis secretory protein 4 (HE4) detection in diagnosis of gynecological adnexal masses. Methods A total of 219 patients with gynecological adnexal masses treated in the hospital from January 2012 to April 2015 were selected as study objects, then they were divided into benign lesion group (191 patients) and malignant lesion group (28 patients) according to postoperative pathological diagnosis; 405 women after healthy physical examination during the same period were selected as control group; ELISA was used to detect serum HE4 levels in the three groups, then the results were compared and analyzed. Results The level of serum HE4 in malignant lesion group was statistically significantly higher than those in benign lesion group and control group (P〈0. 05 ) . According to ROC, the cut-off value of HE4 detection for adnexal malignant lesions among premenopausal women was 61.7 pmol/L, the sensitivity and specificity were 85.7% and 95.3% , respectively; the cut-off value of HE4 detection for adnexal malignant lesions among postmenopausal women was 72. 9 pmol/L, the sensitivity and specificity were 78.6% and 91. 2%, respectively; the areas under ROC were 0. 959 and 0. 833, respectively. Conclusion Detection of serum HE4 is a good indicator in differential diagnosis of benign and malignant gynecological adnexal masses.
