摘要
目的探讨肿瘤患者口服化疗药物依从性的现状并分析影响依从性的相关因素。方法采用描述性研究,运用自行设计的调查表和Morisky依从性问卷,调查180例口服化疗药物的肿瘤患者的相关资料及依从性现状,并进行统计学分析。结果 180例口服化疗药的肿瘤患者中,依从性好的为54例(30.00%),依从性差的为126例(70.00%);单因素检验显示文化程度、社会支持、化疗不良反应、经济条件、口服化疗相关知识、医疗费用支付方式、求医条件不同的患者,其依从性的差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,影响患者口服化疗药物依从性的独立因素为:缺乏口服化疗相关知识(OR=4.46)、化疗不良反应(OR=3.82)、不良心理反应(OR=3.24)、经济条件差(OR=2.97)和缺少社会支持(OR=2.86)。结论口服化疗药物的肿瘤患者存在依从性差的问题,主要表现为不注意服药、忘记服药和自行停药;缺乏口服化疗相关知识、化疗不良反应、不良心理反应、经济条件差和缺少社会支持等均会影响患者的服药依从性。医护人员应针对这些因素采取积极措施提高患者的服药依从性,以保证药物疗效和治疗效果。
Objective To investigate the status of the adherence of oral chemotherapy for cancer patients,and to analyze the related effect factors.Method The related data and the current situation for the adherence of oral chemotherapy among 180 cancer patients was descriptively investigated by self-designed questionnaire and Morisky adherence questionnaire.The differences factors related to good and poor adherence of the cancer patients was analyzed and compared.Result 54 of the 180patients（30%）reported good adherence and 126patients（70%）reported poor adherence.Single factor analysis showed that the educational level,social support,economic conditions,adverse reaction of oral chemotherapy,oral chemotherapy related knowledge,payment of medical expenses,the resort to health services had statistical significance to the adherence of oral chemotherapy of the cancer patients（P〈0.05）.Multivariate logistic regression analysis showed that the lack of related knowledge（O=4.46）,adverse reaction of chemotherapy（OR=3.82）,adverse psychological reaction（OR=3.24）,poor economic conditions（OR=2.97）and lack of social support（OR=2.86）were independent factors affecting the adherence of oral chemotherapy for cancer patients.Conclusion Poor adherence is a vital problem for cancer patients receiving oral chemotherapy,they may had not paying attention to medication,forgot their medication and withdrawal the medication by themselves;lack of oral chemotherapy related knowledge,the adverse reaction of chemotherapy,adverse psychological reactions,poor economic conditions and lack of social support can influence the adherence of cancer patients,health care staff should take effective measures to improve the adherence of these patients,so as to ensure the effect of drug efficacy and treatment.
