DebakeyⅠ型主动脉夹层患者术前低氧血症对预后的影响 被引量：4

摘要 目的:总结主动脉夹层患者术前低氧血症的发生情况,评估其对预后的影响。方法:回顾性分析我院近年行手术治疗的63例DebakeyⅠ型主动脉夹层患者的临床资料。根据患者术前的氧合指数(PaO2/FiO2)分为两组,≤200 mm Hg为低氧血症组,>200 mm Hg为非低氧血症组。比较两组患者术前症状、术中氧合情况及预后指标的差异。结果:低氧血症患者术前易出现精神症状,且具有主观症状不明显、对缺氧相对耐受的特点。低氧血症组术中氧合情况进一步恶化,术后呼吸机辅助通气时间、ICU滞留时间、术后住院时间、术后低氧血症发生率显著高于非低氧血症组。两组术后再次插管率、非呼吸系统并发症发生率及病死率比较差异无显著性。结论:低氧血症患者术前有其特殊临床表现,术后呼吸机辅助通气时间、ICU滞留时间、术后住院时间延长,增加了术后低氧血症发生率,但并未对术后再次插管率、非呼吸系统并发症发生率、病死率等产生明显影响。
